Participants will specifically assume all risks of injury arising out of his or her presence on or about the premises, or his/her use or intended use of equipment or facilities, or his/her participation in the activities at The Tumble Barn, LLC. I am fully aware that any activity involving motion, height or athletic activity creates the possibility of injury, including but not limited to scrapes, cuts, bruises, sprains, breaks, muscle injuries, concussions, or other serious injuries. I fully understand and accept these risks as well as that are unknown to me upon the signing of the Agreement that may result in medical assistance, medical expenses, and medical emergencies. I hereby release The Tumble Barn, including its officers, owners, and employees from any liability of any such injury. I further agree to hold harmless, indemnify and defend The Tumble Barn, including its officers, owners, and employees from any loss, liability, damage, or cost incurred by them at any event at The Tumble Barn. This release includes but is not limited to any claims of negligence, dangerous conditions, latent defect, premises liability, code violations, negligent security, failure to warn, vicarious liability, negligent hiring, supervision, maintenance, or improper/dangerous equipment; it is intended to be as broad as permissible under Illinois law. Parents or Guardians signing this Contract assuming the risk and releasing claims as herein stated are doing so on their own behalf and on behalf of the minor child or children they are signing for, including their respective Estates, Guardians, and Representatives.
I /We authorize The Tumble Barn to seek medical emergency assistance in the event of an emergency for my athlete. I understand I am responsible for all costs and payments associated with the subject of medical emergency or medical assistance for my athlete provided by a third party such as urgent care, hospital, emergency room, etc.?
I have voluntarily enrolled my child in a class which is being run by The Tumble Barn. As part of the consideration for my enrollment in this program, I hereby covenant and agree not to hold The Tumble Barn or their agents and employees or any other person connected with The Tumble Barn responsible for any personal injuries, death, or property damage arising from employees or any other person connected with the studio from and any and all claims, demands, and damages of any kind, known or unknown, existing or arising in the future, resulting from or related to personal injuries, death, or property damages arising from our participation in this program.
