I hereby give permission for any and all medical attention to be administered to my/our child/ren, in the event of accident, injury, sickness, etc., under the direction of the physician listed below or at any necessary emergency facility, until such time as I may be contacted. I also assume the responsibility for the payment of any such treatment, acknowledging and affirming that my/our child/ren are fully covered by a medical insurance policy as listed below. I understand that any medical procedures, injuries or health changes will require a medical release from a medical physician to continue dance program. I understand that my/our child/ren are required to be fully covered by medical insurance for the duration of the child/ren’s enrollment in KD Dance Studio Inc.’s programs. I agree to notify KD Dance Studio Inc. within seven (7) days if any of the information listed below changes or if my child is not medically cleared to dance. This release is effective for the period of one year from the date enrolled at KD Dance Studio. All known allergies have been disclosed to KD Dance Studio Inc.

I agree to payment terms set forth by KD Dance Studio, Inc in the Terms and Conditions which are restated here.

Tuition payments to KD Dance are due on the 1st of every month. First months tuition and registration fee are due at time of registration. Tuition will must be paid monthly until the end of the season. Regular dance season is September through May.

Payment options:

First option with NO convenience fees: Make automatic monthly payments from your bank auto pay feature that is directly mailed to: KD Dance Studio, PO BOX 456, Fayette City, PA 15438. You have to set this feature up through your online banking NOT the parent portal. Checks mailed from your bank must be post marked by the 1st of each month or your account will be assessed with a late fee. Please mail tuition 5 days prior to the 1st each month to avoid late fees.

Second option with convenience fees: You may choose to use a debit or credit card ONLINE in your parent portal and pay fee stated on the website. Your card must be kept up to date inside the parent portal in order to avoid any additional charges for declined payments.

Additional FEES:
There is a $25.00 late fee charged if your payment/check is not received by the 7th day of the month.
There is a $10.00 processing fee if your Card on File (COF) is declined.
There is a $35.00 return bank check fee.

A student will not be permitted to attend class if tuition & fees are not paid.
Any payments including tuition, costumes and recital tickets are NON-REFUNDABLE & NON-TRANSFERABLE.

I give full rights to the KD Dance Studio, Inc. to use photos and video images of me/us or my/our child to use for promotional and advertising purposes of the studio and it’s affiliates. Photos and videos will be used in brochures, websites, advertisements, and other promotional material created by the studio. Photos may appear with or without names in press releases and other print advertising. Current dance families are permitted to share images of dance class, community events or any social gathering that involves KD Dance Studio, Inc.

By signing, I acknowledge that I have read the terms and conditions document provided inside the portal by
KD Dance Studio, Inc. I understand the Dancewear Requirements, Cell Phone/Smart Watch/Device Policy, Physical Expectation, Behavioral Expectations and Dismissal Policy and how they apply to my student, myself and anyone accompanying them to class, rehearsal, recital, or any studio related activity.

I, hereby enroll my/our child/ren into the KD Dance Studio, Inc. school of dance and give permission to KD Dance Studio, Inc. to train, supervise, and assist our child/ren in the art of dance. I/We understand that I/We am/are enrolling my/our child in a program of physical activity and have agreed that my/our child is in good physical condition and does not suffer from any disability that would prevent or limit participation in this dance program, further acknowledging that the art of dance requires physical exertion and risks of injury. I/We further acknowledge as part of the dance instruction an instructor, employee, or agent of KD Dance Studio, Inc may be required to touch my/our child for the purpose of instructing various dance motions and routines. I/We hereby waive any claim or cause of action against KD Dance Studio, Inc., its instructors, employees, agents and assigns from any injury arising out of any and all activities conducted at the dance studio, recital, or competition site. I take full responsibility for any students not enrolled that accompany my registered student(s) in "bring a friend" or trial dance classes.
