Liability Release Form and Assumption of Risk - Dancer


PARTICIPATION - I realize that participation in dance performance, training and other activities could result in some possible personal injury. Despite precautions taken by Wainwright Dance Academy/Wanna Dance and its employees, agents or instructors, accidents and injuries may occur. By signing the registration form, I assume all risks related to the participation in dance performance, training and other activities operated by the Wainwright Dance Academy/Wanna Dance.

I agree to release from responsibility Wainwright Dance Academy/Wanna Dance including all staff members, instructors and facilities used from any cause of action, claims or demands now and in the future. I/We will not hold Wainwright Dance Academy/Wanna Dance liable for any personal injury including: scrapes, bruises, cuts, sprains, fractures, broken bones, concussions or death or any personal property damage/loss, which may occur while participating in Wainwright Dance Academy/Wanna Dance’s 2024/2025 dance season. I hereby release and agree to hold Wainwright Dance Academy/Wanna Dance harmless from, and waive on behalf of myself, my heirs and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of Wainwright Dance Academy/Wanna Dance, or that may otherwise arise in any way in connection with attending and/or participating in the Wainwright Dance Academy/Wanna Dance dance season. I understand that this release discharges Wainwright Dance Academy/Wanna Dance from any liability or claim that I, my heirs, or any personal representatives may have against Wainwright Dance Academy/Wanna Dance with respect to bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, my attending Wainwright Dance Academy/Wanna Dance 2024/2025 dance season. This liability waiver and release extends to the entirety of Wainwright Dance Academy/Wanna Dance with all owners, partners and employees.


PHOTOGRAPHY/VIDEO - I give permission for my son/daughter’s photographs/videos to be used for publicity and advertising by Wainwright Dance Academy/Wanna Dance. This includes, but is not limited too, websites, print and social media platforms.

Studio Policies

Season + Fees

The dance season begins Monday September 9th and runs until approximately May 25th. Tuition may be payed in full by September 9th 2024 or by post dated cheques dated the first of each month. (September-May), or by online payments through Dance Studio Pro (convenience fee may apply). Etransfers will be accepted for full season tuition payments only. Etransfers are not being accepted for monthly tuition in 2024/2025.

In the event of an NSF return on a post dated cheque a $50 fee will apply in addition to the amount of the NSF cheque.

A late fee of $10 is applied to each dancers account ins the event monthly fees are five (5) or more days late.

There are no refunds on classes missed, however if an equivalent class is offered you are welcome to make up a missed day in that class. The studio must be notified in advance if you plan on attending a class not in your registered time. Dancers are not able to make up classes of a different level, this can disrupt the regular class for other dancers.

There will be a minimum of twenty eight (28) classes during the season. Make-up classes will be scheduled in the event of cancellations due to inclement weather, competition travel and instructor illness, etc if less than twenty eight (28) classes will have been completed by recital. Class cancellations will be posted on the studio Facebook page “Wainwright Dance Academy - 2024/2025 Dance Families", the studio Instagram, and by Remind 101.

*The owner/director, staff and faculty of the Wanna Dance Canada/Wainwright Dance Academy have the right to refuse lessons to any child who's tuition has not been paid.

Attendance

Regular attendance is key to your child's progress and the progress of the class as a whole, please keep missed classes to a minimum. Please let the studio know if your child will be absent, so we can notify your teacher. Please

Communication

In an effort to protect our environment all important studio communication will posted on our studio Facebook pages instead of handing out paper memos. You can also search “Wainwright Dance Academy - 2024/2025 Dance Families” and request to join our group.

Wainwright Dance Academy has a group text feature through www.remind.com. Sign up by sending a text to (438) 800-2319 with @wdadan in the body of the message to stay updated.

The studio also has a bulletin board in the waiting room where all important information is posted. Please check this regularly.

For events such as “The Nutcracker”, dance exams, and competitions a google form may be used for permission/registration.

Please feel free to contact the studio by phone or email whenever the need arises. Most questions/issues can be most effectively resolved by email. Teachers only have a few moments between classes, if you need to ask a quick question we ask that you respect that there are many students paying to work with a teacher and be as brief as possible. If something needs more than a few moments please contact the studio to arrange a phone conference and we can have your child's teacher call you. The fee is $20 per parent/teacher conference.

Any time it is necessary to speak to a teacher/staff member please remember to be polite and respectful. Confronting a teacher/staff member in a rude/disrespectful manner or engaging in any kind of verbal or physical abuse will result in the immediate dismissal of your child(ren) and any remaining tuition will not be refunded.

Costumes

A non-refundable costume deposit of $75/class is due at registration, invoices for any remaining costume balances will be sent with a due date via Dance Studio Pro. Any costumes not paid for in full by the due date on their invoice will not be ordered, dancers may not participate in recital without a costume. If a costume needs to be ordered after the class order due to late payment there will be a late fee of $20/costume as well as being responsible for any extra shipping charges. Costumes ordered after the class order can not be guaranteed to arrive on time for recital/competitions.

If a dancer withdraws from class after costumes have been ordered parent/guardian is responsible for any remaining balance on that costume.

Parents/Guardians are required to measure their child and submit their sizes stating which costume size they would like ordered for their child to the studio via Dance Studio Pro. Costume alterations are the responsibility of the parent/guardian.

Instructions for measuring and costume ordering will be sent via email/Dance Studio Pro early in the season.

*Dancers taking two of the same ballet class/week will only require one costume.

Class Placement

Our teachers want every child to reach their full potential, to make this possible final placement of a student is decided by the teachers/studio director. Age limits set for classes are a guideline, every child progresses at their own pace. If a student is above or below the level of a class they are placed in the studio will let you know which class(es) are suited to their level. In the interest of safety a student will not be allowed to remain in a class that is above their current level.

Behavior/Classroom Etiquette

We want every child to have a safe, enjoyable, and educational experience at our studio. Our staff will treat you/your children with courtesy and respect and expect you/your dancers to show the same attitude toward the staff/fellow dancers. Please review the following simple rules with your child(ren):

Pay attention in class - Talking with friends during class disrupts the whole class and makes it difficult for the teacher to communicate with their students. If a student is being uncooperative and a disruption to the rest of class they will be asked to leave. If a student is asked to leave class three (3) times, they will be dismissed from the studio and any remaining tuition will not be refunded.
Cell phones/iPods/iPads etc are not permitted in the classroom.
Gum/food/drinks are not permitted in the studios and must be kept in the waiting areas. Please keep our studio clean and dispose of your trash properly.
Water bottles with secure lids are permitted in the studio. (Water ONLY)
Students should go to the washroom before class. People entering/exiting the room throughout the lesson is disruptive. *We understand that this may be difficult for some of our littlest dancers and request that the parents/guardians of our 2-4 year olds remain near the studio for the duration of their classes just in case.
Teachers will enforce dress code. A student arriving without the appropriate hair/uniform will not be permitted to participate in class.


Punctuality

Students should arrive with enough time to change and begin class on time. A student arriving after class has started will not be able to enter the studio as this interrupts the dancer's entire class. There are no refunds on classes missed due to late arrival.

Late dancers will be permitted to participate in extenuating circumstances at the director's discretion. If it is known in advance a student will be late for a class please notify the studio as soon as possible.

If a student needs to leave a class early it is important that they notify their teacher before class begins.

Studio Spirit

Many of our dancers consider WDA to be their second home. As a studio, we strive to create a warm, welcoming, and supportive atmosphere for dancers of all ages and ability levels regardless of the amount of time they spend in the studio. It is the collective responsibility of WDA staff, instructors, our dancers, and our dance parents to foster a positive environment for our students.

If a dancer or dance parent is not capable of discussing/interacting with others, regarding WDA matters/staff/other students, whether on WDA property or otherwise in a kind and respectful manner/in a way that is detrimental to the positive environment of WDA they will be dismissed from the studio without refund.

WDA takes concerns very seriously and requires them to be addressed via the appropriate channel to quickly rectify any problems that may arise. Concerns should be brought directly to the studio director in writing via dancewainwright@gmail.com under no circumstances should concerns be addressed via text message. If a concern is not brought forward the studio director cannot take steps to rectify it. A dancer will never be penalized for a parent asking a question/addressing a concern.
