I understand my child will be participating in classes with Center Stage Dance Academy faculty. I agree I will not hold any Center Stage Dance Academy staff or faculty responsible for personal injuries or loss or damage to personal property. I understand that as a parent, it is my responsibility to inform faculty and staff of any limitations my child has.
