CSDC Policy and Procedure Agreement:
I understand and agree to abide by the CSDC policies and procedures detailed in the Center Stage Dance Company Handbook that is available to view online.
I give CSDC permission to take and utilize photos of my child for advertising purposes on social media. (If you would like to opt out of photos, please text the studio.)
In no event shall Center Stage Dance Company, its owners, employees, or other representatives be liable for damages including, without limitation, any direct, indirect, incidental, special, exemplary, punitive, or consequential damages sustained out of attendance at this dance studio, competitions, conventions, recitals, and/or rehearsals.
This agreement is in effect through August 2027.
