I am affiliated with SEDC and am at least 18 years old.

I hereby grant Studio E Dance Company and its representatives the right to take photographs and/or videos of my student during my participation in dance classes, rehearsals, performances, and related events. I authorize Studio E Dance Company, its assigns and transferees to copyright, use, and publish the same in print and/or electronically.

I agree that Studio E Dance Company may use such photographs and/or videos of me with or without my name and for any lawful purpose, including but not limited to publicity, advertising, social media, and web content.

I waive any right to inspect or approve the finished product wherein my likeness appears.

I recognize and understand the risks of physical injury inherent to dance and dance training and I fully assume those risks. I hereby release SEDC, LLC, its employees, and instructors from all liability of damage, injuries, or expense due to injuries or illnesses that may occur to my son/daughter during their attendance or participation in classes, rehearsals, workshops, performances, or other dance activities sponsored by SEDC, LLC. I acknowledge that certain types of injuries are common and inherent in dance-related activities, and hereby agree that I, my child, our assignees, heirs, distributes, guardians and legal representatives will not make a claim, demand, or cause of action against SEDC, LLC, its employees and instructors employed by or contracted by SEDC, LLC. They are not liable for personal injury or loss of/damage to personal property while attending SEDC, LLC activities or participating in off-premises activities sponsored by SEDC, LLC.
