Placement for all classes is at the discretion of the teacher and studio owner.
Placement is based on ability and adherence to studio rules and regulations.
A meeting will be scheduled with parents to discuss if class changes need to be made.

In the event of injury or accident, by clicking the Medical Checkbox below, I hereby authorize my child(ren) to receive any emergency medical attention deemed necessary while present at Creative Outlet Dance Academy if parents and emergency contacts cannot be reached by telephone. I also hereby release, indemnify, and hold harmless Creative outlet and its faculty members from all liability or claims. By clicking the Emergency Services checkbox below, I hereby certify that I have read and understand Creative Outlet's Emergency Services Agreement Policy as stated above. I further certify that I will uphold the terms of this agreement for the present dance year.

Students with accounts past due longer than 90 days will not be allowed to participate in class

A $10.00 late fee will be automatically charged to all accounts that are 15 days past due.

There is a $25 returned check fee for all checks returned from the bank.

Classes, costume fees, and performance fees are non-refundable.

You must notify the studio 30 days in advance and in writing, email creativeoutletda@gmail.com if you intend to withdraw your child from dance class. You will be financially responsible for tuition and any late fees if you fail to notify us.

We have a zero-bullying policy in the studio.
Any issues should be brought to the attention of the instructor and studio owner immediately.
Please discuss the expectations for class prior to attendance and read over the Manners for Dancers.
We want everyone to have a positive studio experience.

I hereby consent to the photographing of my child(ren) named above, and the recording of his/her/their voice(s) and the use of these photographs and/or recordings singularly or in conjunction with other photographs and/or recordings for advertising, publicity, commercial or other business purposes. I understand that the term "photograph" as used herein encompasses both still photographs and motion picture footage. I further consent to the reproduction and/or authorization by Creative Outlet Dance Academy to reproduce and use said photographs and recordings of the above named child(ren) for use in all domestic and foreign markets. Further, I understand that others, with or without the consent of Creative Outlet Dance Academy should use and/or reproduce such photographs and recordings. I hereby release Creative Outlet Dance Academy, and any of its associated or affiliated companies, their directors, officers, agents, employees and customers, and appointed advertising agencies, their directors, officers, agents and employees from all claims of every kind on account of such use.

Tuition and Fees are due the first of the month.

For your convenience, yearly tuition is broken down into ten equal monthly installments. It is an average of classes per month. There will be no refunds for classes missed or scheduled holidays. We follow the Baldwin County Public School calendar and will follow their recommendations for weather or emergency closings.
