I, the parent/guardian of the student registered on my account, hereby consent to my child participating in interviews, photos, and videos to appear on Social Media, including Tik Tok, Facebook, Instagram, the website sarahs-dance.com and all media types for Sarah's School of Dance.

If you prefer to not have your child’s picture posted on social media please send an email to sarahs.dance5678@gmail.com to request this setting to be changed.

I have received or will obtain a copy of Sarah's School of Dance policies and will take the responsibility to carefully read and follow the rules and policies therein. I understand that Sarah's School of Dance does not give credit and/or refunds for class(es) missed due to holiday, vacation, illness, weather, etc. I further understand that there are specific risks of physical or property damages, losses, or injury that may result from my or my child’s participation with Sarah's School of Dance, and I voluntarily assume the risks associated with such participation.
