All care will be taken to see that no injuries are incurred during dance class, however, it is a physical activity and as such, the student/guardian must realize that the student needs to be in good physical health. If they have any concerns, they need to check with their healthcare provider before beginning this class.

If the student should be injured, they and their parent/guardian will in no way hold the Happy Feet Dance Company responsible and will not hold the instructors responsible.

I hereby agree to release Happy Feet Dance Studio and its staff from any and all liabilities due to any accidents or injuries that may occur while in class or on the premises.
