Liability Waiver
I/we realize that participation in dance classes and activities could result in some possible personal injury. Despite precautions being taken by the studio, accidents and injuries may occur.
I/we will not hold Aspire Performing Arts Studio, liable for any personal injury including: scrapes, bruises, cuts, sprains, fractures, broken bones, concussions or death
or any personal property damage/loss, which may occur on the premises before, during or after classes.
By signing this release form, I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces used by Aspire Performing Arts Studio Inc.

Medical Attention
In case of physical injury or medical emergency, I hereby authorize Aspire Performing Arts Studio Inc. to make necessary arrangements to transport myself or my child to a medical treatment facility as necessary. All such transportation and medical treatment will be at my sole cost and expense.

Protection of Property
I understand and agree that it is my sole responsibility to safeguard my personal property while attending or participating in any classes, rehearsals, workshops, or performances. I hereby release Aspire Performing Arts Studio Inc., employees and dance teachers from all liability for loss or damage to my personal property while attending or participating in classes, rehearsals, workshops, or performances. I also agree to abide by any rules, regulations and policies set forth by Aspire Performing Arts Studio Inc.




Annual class fees is based from September to end of May.

Payments are due on the 9th of each month from September to May.
1. Upon registration there is a $25 + tax non-refundable family registration fee
2. Costume fees are $25 + tax per costume
3. Payment is accepted via e-transfer, cash, debit or credit card
4. Students who wish to discontinue classes, must confirm at the front
desk before the 9th of the month
6. If payment is not up to date unfortunately the dancer will no longer
be able to participate in class.


CLASS CANCELLATIONS

The studio reserves the right to cancel classes due to inclement weather or other emergencies.
There will be no refunds for cancellations due to weather as this is out of our control.
There will be a refund or class replacement for classes that are cancelled due to instructor illness or emergencies.


RECITAL POLICIES

1. Parents do need to purchase tickets for the year end dance show.
2. Children under the age of 5, can attend for free however will not receive a ticket as if the show is sold
out then they will be required to sit on the adult's knee.
2. All family accounts must be paid in full before a child can participate in the show.
3. If child misses the last 4 weeks of class then they will not be allowed to participate in the show as it would be too difficult for the dancer to know the choreography.


PHOTOGRPAHY/ VIDEO

Aspire Performing Arts Studio Inc. reserves the right to use photographs and videos taken during classes, workshops, performances, or other affiliated events for the purposes of instruction, advertising and promoting the studio and its programs. Students, or parents of students who are minors, who do not wish to comply with this policy must notify the studio prior to participation in class.
