I recognize and understand engaging in any physical exercise, class or activity, including dancing, may result in injury to the participant. I understand that my child will participate in such activities at his/her own risk. I accept full responsibility for the cost of treatment and payment of any and all medical bills. I agree that we are voluntarily participating in activities and use of The Dance Academy of the White Mountains (and it affiliates, employees, agents, representatives, successors and assigns) from any and all claims or cause of actions (known or unknown) arising out of its negligence.
I certify that a physician has examined my child and I have been advised by said physician that my child is approved to engage in the activities offered by The Dance Academy of the White Mountains.
I understand that if my child/children does not abide by the instructions, rules and regulations of The Dance Academy of the White Mountains, as determined in its sole and absolute discretion, he/she is subject to immediate removal from the premises.
I understand that dance and tumbling classes may include,without limitation, dancing with props, stretching, exercise bands and balls, mats, barre work, across the floor combinations, dance routines in the center, and other related activities. I further understand that all of the activities of the dance class involve some degree of risk of strain or bodily injury. The Dance Academy of the White Mountains is not responsible for personal property.
I agree to be responsible for reading studio correspondence and respecting deadlines, if applicable.
I hereby grant permission of The Dance Academy of the White Mountains to use in perpetuity, my child's images from photographs, film, videos, and other media for promotional purposes or materials in marketing. This may include, but not limited to use in brochures, flyers, television, internet, exhibitions, social media and web broadcast.
I certify that I am eighteen (18) years of age or older, and that the statements contained in the Parent/Guardian Consent and Liability Release Form are accurate.
I have carefully read and will take the responsibility to follow the rules and policies therein of The Dance Academy of the White Mountains. I understand failure to follow rules and policies will be grounds for immediate dismissal with no refund.
