Participation & Assumption of Risk
I understand that participation in dance classes, rehearsals, performances, competitions, workshops, camps, and other Premier School of Dance activities involves physical activity and carries inherent risks of injury. I voluntarily allow my child (or myself, if over 18) to participate and assume those inherent risks.
Medical Authorization
In the event of illness or injury, I authorize Premier School of Dance staff to seek emergency medical treatment if I cannot be reached immediately. Every reasonable effort will be made to contact me first. I understand that I am solely responsible for all costs related to such medical treatment and any related medical transportation.
Release of Liability
To the fullest extent permitted by law, I waive, release, and hold harmless Premier School of Dance, its owners, instructors, staff, assistants, volunteers, and affiliated facilities from all claims, known or unknown, arising from participation in studio activities, except where caused by gross negligence or intentional misconduct.
Health & Participation
I certify that my child is physically able to participate and will notify the studio of any medical conditions, allergies, injuries, or restrictions that may affect safe participation.
Photo & Video Release

I DO NOT authorize the use of photographs or videos of my child for promotional purposes.

Parent/Guardian Agreement
By signing below, I acknowledge that I have read, understand, and voluntarily agree to the terms of this waiver and release. This agreement applies for the entire 2026–2027 dance season.
