I give permission for my child's photograph to be used for advertising & achievement purposes. I understand that no other information will be distributed. I understand that fees are due for classes & competitions provided and entered (all missed lesson fees will still be due) and will finalise fees within the 21 day Invoice period. Failure to pay fees in the time requested may result in the engagement of a mercantile agent. Any and all associated costs for this action will be your responsibility as parent or legal guardian of the participating child. *No student will be able to re-enrol for classes until overdue fees are paid. During the season - students may be asked to sit out if fees get behind. I understand that if we decide to withdraw from classes at Attitude Dance Centre Pty Ltd trading as Attitude Dance & Cheer we must give 2 weeks notice in writing to accounts@attitudedancecentre.com.au I understand that there is a possibility of physical injury by participating dancing & cheerleading classes & performances, and that if required teachers or coaches will call emergency assistance if needed and that all expenses of such are my responsibility. Students behaving in an unsafe manner will be asked to sit out until their behaviour is appropriate again.
