I have read the Dance Force-1 policies listed below and will take the responsibility to carefully follow the rules and policies therein. I understand that Dance Force-1 does not give credit and/or refunds for class(es) missed due to holiday, vacation, illness, weather, etc. I further understand that there are specific risks of physical or property damages, losses, or injury that may result from my or my child’s participation with Dance Force-1, and I voluntarily assume the risks associated with such participation.

Policies and Procedures

Please read the tuition and make-up policies below carefully. Your signature below acknowledges you have read and understood them.

Child's Safety – I understand I am responsible for my child's behavior and safety while on Dance Force-1 premises, including parking lots, bathrooms, lobby, etc.

Tuition – Tuition is due the first class of each month. We accept cash, checks and credit cards. Please make checks payable to Dance Force-1, and identify your child's name in the memo section of the check. If your last name is different than your child's, please put your child's first and last name on the check. There will be a $25 charge for all checks returned for insufficient funds. Dancers will not be allowed to participate in classes if tuition is unpaid. There will be a $15 Late Fee accessed on the 15th of each month for past due balances. All registration fees, tuition, costume deposits, camps, and fees associated with dance team are non refundable. The only time tuition is prorated is when a new student joins a class after the session has started. Each student is enrolled in class indefinitely. To withdraw from a class, 30 day written notice is required and needs to be on file before the first of the month when tuition is assessed.

Drop off/pick up policy – I understand that it is my responsibility to drop my child off and pick them up on time. Dance Force-1 cannot keep children in their care after class. Any child left 10 minutes after closing will be assessed a $1 per minute charge.

Medical conditions – Please make sure the instructor is aware of any medical conditions, recent injuries or surgeries.

Attendance – I understand that if my child has extreme tardiness (15 minutes late for class) and or absenteeism (3 consecutive absences), he or she may be dropped from the class(es) and recital performance without refund. Dancers may attend a Make-up class for an absence within 7 days of the absence. Make-up classes do not count toward perfect attendance.

Release of liability, waiver of liability – Assumption of full responsibility for all risks of bodily injury, death or damages.

As a parent or legal guardian, I hereby consent to his or her participation in or all the programs offered by Dance Force-1. I understand that participation in dance and any and all other activities at Dance Force-1, may result in unavoidable injuries including, but not limited to, muscle or other soft tissue strains, sprains and tears, broken bones, and severe injuries such as paralysis or even death from various causes, known and unknown, which include but are not limited to the height of the body during certain movements, rotation of the body, and movement of the body, and a unique environment. I am fully aware of the inherent risks in dance, and all other activities offered by Dance Force-1.

In consideration for allowing my child to participate in activities offered by Dance Force-1, I, my heirs and assigns, next of kin, and all others acting on my behalf agreed to waive any and all rights, claims, damages, actions, causes of action or suits of any kind or nature whatsoever which I have or my child has against Dance Force-1, or any agent, employee, representative or other acting on their behalf for any injuries suffered as a result of engaging in those activities offered by Dance Force-1. It is also my intent to release Dance Force-1, April Hoose, and any agent, employee, representative or other acting on their behalf from liability for ordinary negligent conduct, which may occur in the future.

I give Dance Force-1 permission, in connection with any photographs taken during Dance Force-1 activities of me, my family, my child named herein, the right to use and reuse, in any manner at all, photographs in whole or in part, either by themselves or in conjunction with other photographs, in any medium and for the purposes whatsoever, including without limitation, all promotional and advertising uses, and other trade purposes. I hereby release Dance Force-1 and its owner from any and all claims, actions, and demands arising out of or in conjunction with use of said photographs.

Further, in case of accident, injury or sudden illness, I authorize any first aid or emergency medical care that may become necessary for my child while at Dance Force-1. I also authorize that my child may be transported to a local medical facility. If I cannot be reached in an emergency, I hereby grant permission for my child to receive all appropriate medical treatment necessary. I agreed to provide health insurance for the minor child or guarantee payment of any medical expenses incurred as a result of training, performing, or participation in activities with Dance Force-1. By executing this document I hereby assume on behalf of my child, all risk of injury or loss to which he or she may be exposed.

This acknowledgement of risk and waiver of liability has been read by me and understood completely and signed voluntarily.

I am 18 years of age or older.

I HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING IN THIS ACTIVITY OR EVENT, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault.

I certify that I am physically fit, have sufficiently prepared or trained for participation in the activity or event, and have not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude my participation in this activity or event.

I acknowledge that this Accident Waiver and Release of Liability Form will be used by the event holders, sponsors, and organizers of the activity or event in which I may participate, and that it will govern my actions and responsibilities at said activity or event. In consideration of my application and permitting me to participate in this event, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:

I HEREBY WAIVE, RELEASE, AND DISCHARGE Dance Force-1, LLC. and all divisions thereof of any and all liability and responsibility for injuries, sickness, pandemics, accidents, natural disasters and/or acts of God incurred during participation in and/ or instruction of camps, intensives, private instruction, choreography or any activity I may participate.

I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from this event, THE FOLLOWING ENTITIES OR PERSONS: Dance Force-1 LLC and/or their directors, officers, managers, employees, volunteers, representatives, and agents, the activity or event holders, activity or event sponsors, activity or event volunteers;

I INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this waiver, release and registration form from any and all liabilities or claims made as a result of participation in this activity or event, whether caused by the negligence of release or otherwise.

The accident waiver and release of liability shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.

I CERTIFY THAT I HAVE READ THIS DOCUMENT, AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT OF MY OWN FREE WILL.

PARENT / GUARDIAN WAIVER FOR MINORS (Only if student is under 18 years old)

The undersigned parent and/or natural guardian does hereby represent that he/she is, in fact, acting in such capacity, has consented to his/her child or ward’s participation in the activity or event, and has agreed individually and on behalf of the child or ward, to the terms of the accident waiver and release of liability set forth above. The undersigned parent or guardian further agrees to save and hold harmless and indemnify each and all of the parties referred to above from all liability, loss, cost, claim, or damage whatsoever which may be imposed upon said parties because of any defect in or lack of such capacity to so act and release said parties on behalf of the minor and the parents or legal guardian.
Electronic Signature Agreement. By selecting the "I Accept" checkbox, I am signing this agreement electronically. I agree this electronic signature is the legal equivalent of my manual signature on this agreement.

I have had sufficient opportunity to read this entire document. I have read and understood it, and I agree to be bound by its terms.
