Release of Liability
I/we (the dancer and parent/guardian) realize that participation in dance classes and activities could involve some possible
personal injury. Despite precautions, accidents and injuries may occur. By signing this release form, I/we assume all risks
related to the use of any and all spaces used by Guardian Training Systems, LLC.
I/we agree to release and hold harmless Guardian Training Systems, LLC including its teachers, dancers, staff members,
and facilities from any cause of action, claims, or demands now and in the future. I/we will not hold Guardian Training
Systems, LLC liable for any personal injury or any personal property damage, which may occur on the premises before,
during or after classes. Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our
behavior in addition to any damage I/we may cause to the facilities utilized by Guardian Training Systems, LLC.
I understand that Guardian Training Systems, LLC are licensed, accredited and insured organizations. In the event that I/
we should observe any unsafe conduct or conditions before, during or after my/our classes, I/we agree to report the unsafe
conduct or conditions to the instructor or staff member as soon as possible.

Release of Liability

related to the use of any and all spaces used by Guardian Training Systems, LLC, DBA Unity Cheer.
Acknowledgment and Assumption of Risk:

I, the undersigned, understand and acknowledge that participation in cheerleading activities at Guardian Training Systems, LLC DBA Unity Cheer (hereinafter "Unity Cheer") involves inherent risks, including but not limited to, physical injury, emotional distress, and property damage. I voluntarily agree to assume all such risks, whether foreseeable or not.

Waiver and Release of Liability:

In consideration of being allowed to participate in cheerleading classes, practices, and related activities at Unity Cheer, I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby release, waive, and discharge Unity Cheer, its owners, officers, directors, employees, agents, and representatives (collectively, the "Releasees") from any and all claims, demands, actions, or causes of action of any kind, including but not limited to, personal injury, death, or property damage arising out of or related to my participation in any and all activities at Unity Cheer.

Medical Treatment Consent:

I hereby consent to receive medical treatment, which may be deemed advisable in the event of injury, accident, and/or illness during my participation in cheerleading activities at Unity Cheer. I agree to be responsible for any and all costs associated with such treatment.

Media Release:

I grant Unity Cheer the right to photograph, videotape, and/or record me during cheerleading activities and to use my likeness in promotional materials, social media, and other publications without compensation or further notice.

Code of Conduct:

I agree to adhere to all rules, regulations, and instructions set forth by Unity Cheer. I understand that failure to do so may result in disciplinary action, including but not limited to, suspension or dismissal from the program without refund.

Severability:

If any provision of this waiver is found to be unenforceable or invalid, that provision shall be severed from this agreement, and the remainder shall remain in full force and effect.

Acknowledgment of Understanding:

I have read this Waiver and Release of Liability, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing this agreement freely and voluntarily and intend by my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
