A PARTICIPANT is understood to be an account member who is 18 years old and participating in a Keystone Dance Co dance program.

A PROVIDER OF PARTICIPANT is understood be an an account member who is representing a dance who is under the age of 18 and participating in a Keystone Dance Co dance program.

I/we, the PARTICIPANT/PROVIDER OF PARTICIPANT, hereby declare that the information furnished within this form is true, complete and correct to the best of my knowledge and belief. I understand that in the event of any falsification, misrepresentation, omission, incorrect, or any willful dishonest information shall be liable to cancellation/refusal of service without notice or any compensation in lieu thereof.

LIABILITY WAIVER
I/we, the PARTICIPANT/PROVIDER OF PARTICIPANT, realize that participation in KEYSTONE DANCE CO classes and activities could involve possible personal injury. Despite preventative precautions, accidents and injuries may occur. This includes, but are not limited to, all physical and mental injuries occurring on-site and/or through remote virtual activities/initiatives. By selecting "I Agree" on this release form, I/we (the PARTICIPANT/PROVIDER OF PARTICIPANT) assume all risks related to the use and participation of any and all spaces used by KEYSTONE DANCE CO.

I/we, the PARTICIPANT/PROVIDER OF PARTICIPANT, understand and accept by selecting "I Agree", I/we agree to release and hold harmless KEYSTONE DANCE CO, including its teachers, dancers, staff members and facilities used by KEYSTONE DANCE CO from any cause of action, claims, or demands now and in the future. I/we will not hold KEYSTONE DANCE CO for any personal injury or any personal property damage, which may occur on the premises before, during or after classes. Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our behaviour in addition to any damage I/we may cause to the facilities utilized by KEYSTONE DANCE CO.

I/we, the PARTICIPANT/PROVIDER OF PARTICIPANT, understand that by selecting "I Agree" that KEYSTONE DANCE CO is licensed, accredited and insured. In the event that I/we should observe any unsafe conduct or conditions before, during or after my/our classes, I/we agree to report the unsafe conduct or conditions to the Executive Committee, individual instructor or staff member as soon as possible.

MEDIA WAIVER
I/we, the PARTICIPANT/PROVIDER OF PARTICIPANT, understand by selecting "I Agree" that I/we hereby authorize any images or video footage taken, in whole or part, individually or in conjunction with other images and video footage, to be displayed by KEYSTONE DANCE CO on official channels (i.e. Youtube, Facebook, Instagram etc.), and to be used for media purposes including promotional presentations and marketing campaigns. I also authorize the same in connection with any media material created within the KEYSTONE DANCE CO program.

I/we, the PARTICIPANT/PROVIDER OF PARTICIPANT, understand and accept by selecting "I Agree" that I/we waive any rights to the privacy and compensation, which may have in connection with such use of the names and likeness, including rights to any copyright or moral rights that may be created in connection with video connection, editing and promotional there with.
