I hereby grant the Port Perry Dance Academy the irrevocable right and unrestricted permission to use and publish photography and or video images of Student, or in which Student may be included, for any purpose authorized by the Port Perry Dance Academy, including but not limited to: the Port Perry Dance Academy webpage and social media, editorial publications/promotions and video recording advertising.

This includes the right to modify and retouch the images at the discretion of the Port Perry Dance Academy. I understand there will be no compensation to myself. Furthermore, I understand that I will not be given the opportunity to inspect nor approve the finished products or printed/published matter that may be used in therewith. In granting this permission to the Port Perry Dance Academy and its representatives, I am fully and without limitation releasing the Port Perry Dance Academy from any liability that may arise from use of the images.

Absence Due to Illness:
Dancers must stay home if they are sick for the health and safety of our community. If your child has new or worsening symptoms, please keep them home. There are no make up classes or refunds for classes missed due to illness.

Absence Due to Vacation:
There are no make up classes or refunds for classes missed due to personal vacation times. The studio will be closed for March Break, Christmas Holidays and for all Statutory Holidays.

Class Viewing:
Our viewing windows are open upstairs for class viewing for Studios 1 and 2 with a live stream video for Studio 3.

Tuition Payment:
We auto-debit the 15th of every month in 10 instalments from Sept-June. The undersigned is accepting of our auto-pay system and acknowledge that there chosen method of payment will be charged monthly. There is an annual $40.00 registration fee per family and a recital package fee (costume + video) that is debited in the New Year. Registration fees are non-refundable. Credit cards are accepted as a form of payment. Any further pre-authorized fees must be requested to be stopped in writing a minimum of 5 business days prior to the payment being processed.

Late Payments:
A $25.00 late fee will be charged to your next tuition payment for any returned/NSF charges.

Discounts:
A 5% tuition discount is applied to families with 2 or more dancers.

Withdrawal/Cancelation Policy:
A 30 day cancellation notice will be required via email communication when a student is withdrawing from a class.

Cancelled Classes:
We reserve the right to cancel a class due to low enrolment. If applicable, you will be given the option to transfer into another class for any cancelled classes due to this reason.

Inclement Weather:
On occasion, classes may be cancelled due to poor weather conditions or circumstances beyond our control. We will do our best to contact you in advance via email. We are unable to offer refunds or make up classes for the ones cancelled due to these circumstances.

Mandated Studio Closure:
In the event of imposed government restrictions, classes will be maintained via a virtual platform and fee structures will remain in place.

As additional consideration for the students instruction, the undersigned student, parent(s), or guardian(s) of the student hereby releases and waives any and all claims against the Port Perry Dance Academy and all of its employees for any liability for injuries, including personal and bodily injury, to the person or persons related to the Student which may occur while participating in the activities, activities sponsored by, or in the physical building of the Port Perry Dance Academy unless said injuries or damage is caused by the negligence of the Port Perry Dance Academy or any of its employees.

The undersigned represents that the student is in good health and does not have history of a medical of physical condition (unless specified on the Registration form) that would place the student at risk because of his/her condition. The undersigned further acknowledges that the student's instruction involves physical exercise and physical stress that could result in physical injury of the student.

I have read the above waiver and reviewed the policies and terms of the Port Perry Dance Academy and intend to comply with all of their terms.
