ENROLMENT INDEMNITY
Soul Performing Arts Studio (SOUL) takes all reasonable care in the conduct of
the classes it runs. In exchange for my child/children’s participation in classes, rehearsals, photo shoots and/or performances with SOUL, I, the parent/guardian;
1. Have read and agree to SOUL’s Code of Conduct.
2. Acknowledge that myself and my child/ren need to abide by this Code of Conduct
and if not, SOUL has the right to ask a student to leave the class, at any time and
without warning, if they breach or appear to breach any of the guidelines for
creating a safe environment.
3. Will disclose to the teacher before the activity any disability, illness or injury that
may be affected or aggravated by the child/ren participating in a class or activity. I
acknowledge that the teacher is relying on this statement in allowing my child/ren to
take part in the class.
4. Will release the teacher, employees, volunteers, other students and SOUL from all
liability for any claim or demand or damage or personal injury or illness whether
arising as a result of negligence, breach of statutory duty or any other cause of
action sustained by myself or my child/ren, teacher, employees, volunteers or
other students.
5. Accept responsibility for all medical costs which may occur as a result of my or my
child/children’s actions during all activities held by SOUL.
6. Acknowledge that in the conduct of the class or other activity, some physical contact
may occur between my child/ren and the teacher and other members of the class.
7. Acknowledge that if my child/ren miss a class, there are no opportunities for makeup classes.
8. Acknowledge that by participating in SOUL in any capacity, photos of my children
may be used by SOUL as they deem suitable and ethical, unless I advise and opt out.
9. Acknowledge that any person taking any classes offered by SOUL does so entirely at
their own risk and assumes responsibility for all the risks associated with the classes.
10. Will assume the fees associated with my children’s involvement, including any late
fees applied should I fail to pay on time.
I am a legal guardian. By registering my child/ren with SOUL Performing Arts Studio, I declare that I understand and agree to all the above terms and conditions of this
enrolment for my child/ren into SOUL classes.
