THE TALENT ROOM GENERAL WAIVER FORM

1. Acknowledgement of Risk & Release of Liability

I understand that participation in dance, acro, tumbling, conditioning, rehearsals, and other physical activities at The Talent Room involves a risk of injury. I voluntarily assume all risks, known and unknown, including but not limited to falls, sprains, strains, fractures, or other injuries, and I hereby release, waive, and hold harmless The Talent Room, its owners, directors, instructors, employees, volunteers, and affiliates from any and all liability, claims, or demands arising from participation in any activities or presence at the facility.

2. Medical Authorization & Emergency Care

In the event of injury or illness, I authorize The Talent Room staff to obtain medical treatment for the participant, including emergency medical services. I agree to be financially responsible for any resulting medical expenses. I confirm that the participant is in good health and has no condition that would prevent safe participation.

3. Photo/Video Release

I give permission for The Talent Room to photograph, film, or record the participant during classes, camps, events, rehearsals, and performances. These images may be used in promotional materials, social media, websites, and press releases without compensation or further approval. I understand these materials will be used respectfully to promote The Talent Room and its affiliated programs.

4. Behavior & Conduct Agreement

I understand that respectful conduct, kindness, and positive sportsmanship are expected at all times. The Talent Room reserves the right to remove any participant who displays disruptive, dangerous, or inappropriate behavior. No refunds will be provided in such cases.

5. Payment, Refunds & Cancellation Policy

I acknowledge that tuition, fees, and deposits are non-refundable unless otherwise specified in writing. Missed classes due to illness, scheduling conflicts, or injury may not be refunded but may be eligible for make-up classes at the studio’s discretion. I understand that performance and competition fees may have separate deadlines and refund policies.

6. COVID-19 / Communicable Illness Agreement

I understand that despite all precautions taken by The Talent Room, participation in group activities may carry a risk of exposure to communicable illnesses. I agree not to send the participant to class if they exhibit symptoms of illness and to follow all safety protocols set by the studio. I release The Talent Room from any claims related to illness contracted through participation.
