I give my child/children permission to enroll and take class at Diamond Arts. I have read and understand all studio policies for Diamond Arts Wellness Project, and I agree to abide by them while my child takes class at Diamond Arts. Diamond Arts and its instructors are not liable for personal injury or loss of damage to personal property. Diamond Arts also does not assume liability for injury that may occur while studying, dancing, rehearsing or performing at or with the studio. There are always risks involved with physical activity and responsibility must be assumed by the student and their parent or guardian. I also agree to inform the instructor of any physical limitations I may have before dancing at or with Diamond Arts.
I understand that my child has the option of attending class via Zoom if my child is feeling sick. I agree with this option to keep our student body feeling well and out of courtesy of other students and their families.
