RELEASE OF LIABILITY AND PHOTO STATEMENT:
-I understand that participating in dance classes and activities conducted at Studio 48 Dance Co. involve risk and possible injury. I understand that Studio 48 Dance Co. has advised me to seek the advise of my physician before participating in these activities. I agree to indemnify and release Studio 48 Dance Co., staff, and/or volunteers from any and all claims to personal injury sustained now or in the future while participating in activities or traveling to or from its facility and events. In case of a medical emergency for any injury or illness sustained or acquired while at Studio 48 Dance Co. for any class or activity, I give my permission to provide medically necessary treatment for myself or my child with the nearest physician and/or hospital.
-I give permission to Studio 48 Dance Co. to have professional photographers and/or videographers to photograph or videotape myself or my child to be used in any format for any lawful purposes to promote Studio 48 Dance Co. Including Steinies Photique and Dana Freeman Photography.
PORTAL REGISTRATION/TUITION AND DANCE COSTUME POLICIES:
I am so excited about this new portal system, but please understand this system is not to upset any of you, this is to help me and my business to be more successful and be more organized financially, and communicate more efficiently.
-$25 registration fee, per student, and is non-refundable, convenience fees apply.
-$25 Yearly Recital Fee- this is to help with costs of programs, backstage helpers, etc. This is to help keep the standard of excellence that we provide for our shows, and so that all of the parents can enjoy the show as well. This fee will be included in your costume deposit balance, and will be charged once in October, per student.
-New this year, every student will receive a new pair of tights along with their costumes! (if need extras, I will have a sign up form posted in portal at a later date)
-3 or more student discount will receive a 15% discount automatically deducted from your tuition total. (convenience fees still apply)
-If tuition is paid in full for the whole year, you will receive a 10% discount, contact me for info how to do this. (However, no double dips on discounts, only one will be applied, and convenience fees apply to all transactions)
-Studio 48 Dance Co. now only accepts credit/debit card auto-payments through the portal system for tuition and costume payments. Tuition will be deducted at the 1st of every month (if a holiday, 2-3 days later), and costume fee will be deducted twice a year by these dates: (I will post costume totals on your portal in October 20th/February 15th, and then that total will be deducted/charged by these dates) Convenience fee's also apply to pay for the systems transactions which is 3.05%+$.30 per transaction, the 30 cent charge is per student as well. (PLEASE REMEMBER THIS FEE IS TO PAY FOR THE TRANSACTION TO THE DSP MERCHANT COMPANY, NOT ME, I PAY FOR THE OTHER HALF OF FEES THAT I DO NOT CHARGE, INCLUDING BUT NOT LIMITED TO TAX)

Unfortunately Venmo is no longer accepted for tuition/costume payments.
-Cash/Checks will be accepted for dance attire/shoes/merchandise only!
-All tuition and costume fee payments are non-refundable
-Payments that are declined will have a $10 penalty, and your child will not be able to participate in class till the bill is paid for that month. (unless there is communication with me)
-Each current student will receive their schedule at the beginning of August. When registering, please select the current class your dance student is in.

DANCE DRESS CODE:
Students must wear the proper dance attire to participate in class. Dancers must wear a black or pink leotard with tights, leggings, or spandex shorts and the proper shoes for each class. (Our students are required to have a black Dance Attire Package Set that is sold through me.) We only wear pink ballet shoes in the recital, black tap shoes, and black jazz shoes. Absolutely no t-shirts, pants, soccer shorts, or any other clothing that covers dance attire will be allowed. All students must wear their hair up in a ponytail or bun. The dress code is for everyone’s benefit and allows your instructor to watch for correct body alignment such as straight knees, turnout, proper placement of the back, hips, head, shoulders… and assures students will learn proper dance technique. Please make sure your students have all of the appropriate dance shoes for class. You will be able to order shoes/leotards through us if you wish at our Open House! There has been a continuous problem with students not following the dress code. They must be prepared for class each week or they will sit out.
STUDIO 48 RULES:
Attendance Policy: Weekly attendance is mandatory for a dancer’s development. Multiple absences affect not only the dancer but also the entire class, especially while preparing for a recital. Students who miss more than six classes, unexcused, in a year will be dismissed from class permanently. We realize that we all have lives outside of dance, but we have to be fair to those who are truly committed. Please communicate with us, if there are any conflicts, including vacations, sports, etc . Excused absences example: Illness, family emergency, school events for a grade (excluding sports), etc. Sports is always a conflict, which we understand, but missing a month and a half of classes is not acceptable. We are willing to compromise, but have to draw the line somewhere. If there are any questions about scheduling conflicts, please let us know. Thank you!-If wanting to withdraw from a class, please contact studio48danceco@gmail.com. If a student is wanting to withdraw in mid season, tuition will still be charged that following month, on top of no refunds for costumes.
Studio Drop Off: Kindly, we are not a daycare service. It’s important that your children are dropped off no earlier than 5 minutes before their class time and be picked up promptly when class is over. Our instructors are in class teaching, and cannot be responsible for your child sitting in the hallway during class time. It’s for your child's safety. In addition, only students attending class are permitted to enter the building. No parents, siblings or other guests are allowed in the studio. Thank you!
Additional rules:
*PARENTS ARE NOT ALLOWED TO STAY IN THE BUILDING FOR/DURING CLASSES. PLEASE RESPECT THIS RULE. (Our pre-ballet/combo classes, parents are welcome to come in the first week, but after the first 5 min of class, we please ask you to leave the building as the children to listen better when parents are not around, AND AGAIN SAFETY FOR YOUR CHILDREN, this is why this rule is in place, so please respect it).
*A positive work ethic and attitude is expected of all dancers/parents.
*Respect should be shown to all students and teachers. No bullying, gossip, or isolation of other students.
*NO Eating and chewing gum during class is not allowed, bottled water excepted. (please have your student eat before class so my halls don't fill up with crumbs, and food on the dance floor)
*PLEASE HAVE YOUR CHILD USE THE BATHROOM BEFORE CLASS to try to refrain from using during class. We would like the kids to use the restroom either before or after class, so there are less interruptions, and so they don’t miss out on something important. Clearly if they have to go, we will let them, but again, please remind them to use it before class, thank you!
*Cell phones must be off or on “silent” and should only be used during breaks.
