I have read the Zero Gravity Dance policies (as stated below) and will take the responsibility to carefully read and follow the rules and policies therein. I understand that Zero Gravity Dance does not give credit and/or refunds for class(es) missed due to holiday, vacation, illness, weather, etc. I further understand that there are specific risks of physical or property damages, losses, or injury that may result from my or my child's participation with Zero Gravity Dance, and I voluntarily assume the risks associated with such participation.
Electronic Signature Agreement. By selecting the "I Accept" checkbox, I am signing this agreement electronically. I agree this electronic signature is the legal equivalent of my manual signature on this agreement.
I have had sufficient opportunity to read this entire document. I have read and understood it, and I agree to be bound by its terms.
RELEASE, WAIVER and ACCEPTANCE OF RISK: We/I(Parent or Guardian)of hereby acknowledge and agree that in consideration of child(ren) being registered in and participating in dance classes and all associated events including but not limited to workshops, festivals, dress rehearsals and performances (collectively hereinafter Classes) for the 2025/2026 dance instruction season, as follows:
Release:1. We/I do hereby Release and hold harmless ZeroGravity Dance Inc., its insurer, Odessa Wowk, the studio, instructors, members, employees, volunteers, servants and representatives from and against any and all liability, manner of action and actions, cause and causes of actions, suits, claims, demands and costs and expenses of any and every kind and nature and kind at law whatsoever, in respect of death, injury, loss or damage to me or my children's person or property, however because, arising out of or in conjunction with me or my children taking part in the Classes and not withstanding at the same may have been contributed to or occasioned by the negligence of Zero Gravity Dance Inc., Odessa Wowk, the studio, instructors, members, employees, volunteers, servants and representatives.
Acceptance of Risk:2. We/I understand and acknowledge that our/my child's participation in Classes is wholly voluntary. We/I further understand and acknowledge that any and all loss of personal property, including but not limited to cell phones and personal electronics is not the responsibility of Zero Gravity Dance Inc., or Odessa Wowk.
Indemnification:3.We/I agree to indemnify Zero Gravity Dance Inc., and/or Odessa Wowk its insurers, members, employees, volunteers, successors and assigns, from any and all claims, demands or causes of action and all expenses incidental thereto, based upon or arising out of any personal injury, loss of life or property damage or loss caused by or resulting from my child's (children's) acts or omissions during Classes.
ACKNOWLEDGEMENT and AGREEMENT: 4. We/I acknowledge and agree that:
a. A costume is required for every choreography class. Once my child's (children's) costume is ordered, even upon withdrawing from a Class or Classes, we/I will be responsible for the full cost of the costume. We/I am aware of the price guideline and acknowledge and agree that we/I will be responsible for payment in full;
b. An interest charge of 5% per month will be applied to all amounts 30 days past due and acknowledge that we/I will be responsible for paying all amounts and associated interest charge(s) in full by the end of the dance season;
c. There is a $35.00 service charge on any NSF cheques;
d. We/I understand the withdrawal policy and will provide one month written notice in the event my child(ren) withdraw prior to March 1, 2026. We/I acknowledge that we/I are responsible for paying all fees to the end of the notice period. We/I acknowledge there is no withdrawal and no refund of fees after March 1, 2026;
e. Zero Gravity Dance Inc. may use my or my child's(ren's) name and/or photo for advertising or publication purposes;
f. A 10% nonrefundable deposit fee (to reflect the 1st payment) is due at registration;
g. We/I have carefully read this Release, Waiver and Acceptance of Risk agreement and that we/I fully understand same, and we/I are/am freely and voluntarily executing same;
h. With the intent to be legally bound, we/I acknowledge and understand that by signing this Release we/I will be forever prevented from suing or otherwise claiming against Zero Gravity Dance Inc., Odessa Wowk, the studio, instructors, members, employees, volunteers, servants and representatives with respect to any matter arising from the Classes; and
i. This agreement shall be construed in accordance with, and governed by, the laws of the Province of Alberta.

PLEASE READ CAREFULLY
- I understand that by participating in lessons, performances, and any other events organized or participated in by Zero Gravity Dance, my child and I do so entirely at our own risk. I acknowledge there is a risk of becoming exposed to and/or contracting a communicable disease, including COVID-19, despite the preventative measures Zero Gravity Dance has put in place to reduce the spread of communicable diseases.
- I understand and have made my child aware that they are to inform a Zero Gravity Dance staff member immediately if they feel unwell while at Zero Gravity Dance. Should this occur I understand my child will need to be picked up by a parent or guardian.
- I have alerted Zero Gravity Dance of any and all medical conditions and allergies of my child.
- I irrevocably and unconditionally remise, release, and forever discharge Zero Gravity Dance and its directors, shareholders, trustees, officers, employees, partners, and agents and all of Zero Gravity Dance's instructors and volunteers from all and any actions, suits, debts, claims, damages, or liability whatsoever arising out of or related to any loss, damage, illness or injury sustained while in or on Zero Gravity's premises or any other premises attended in relation to dance instruction and performances.
- I acknowledge that agreeing to these terms is a required condition of registration and no exceptions will be made.
CONDITIONS FOR ZERO GRAVITY DANCE STUDENTS
REGISTRATION FEES: $35/person $50/family is due at time of registration. This fee is non-refundable unless we are unable to place the student.
YEARLY TERM FEES: Fees are based on a yearly total, which is then divided into 10 equal payments. Fees are due the 1st of every month. Payment of lesson fees does not guarantee lessons will be held in-person at the Studio. Registration is based on 34 weeks (three or more of these weeks may be used for competitions).
COMPETITIONS: All Part-Time and Full-Time competitive dancers will automatically be enrolled in competitions, which are mandatory. Competition fees are due November 15th and are non-refundable and non-transferrable. The dates will be printed in the fall newsletter. Additional competition fees apply to students wishing to do solos, duets and/or small groups and lines.
COSTUMES: All classes require a costume for the year-end show and competitions, except for Ballet Technique, Conditioning classes and Sessions. Costume deposits are due by October 15th and are non-refundable and non-transferrable.
YEAR END RECITAL: Is in June and all dancers enrolled in full year classes will participate in the year end recital except for Ballet Technique and Conditioning classes. A mandatory event fee will be charged to every dancer registered in a class with a performance group.
Privacy Policy
The privacy of your personal information is very important to Zero Gravity Dance. We will not sell, rent or trade your personal information to any third party. We will take special care to protect the privacy and security of your information.This policy explains our information practices and the way that Zero Gravity Dance collects and uses your information. HOW DOES Zero Gravity Dance COLLECT AND USE MY INFORMATION?
When you register for classes at Zero Gravity Dance, we collect contact information such as guardian names, addresses, phone numbers, emergency contacts, e-mail address and student information such as student names, birthdays, medical conditions, and your comments. Some of the information is required and is noted with a red asterisk on the forms. This information is needed to manage studio business and will not be used or sold for any other purpose, except as described below. Some of the information is optional (NOT marked with a red asterisk). Optional information is requested to help us better understand our customers and is used only as needed for studio business. This information will not be used or sold for any other purposes, except as described below.
WHO HAS ACCESS TO MY INFORMATION?
When you register, you are required to enter an e-mail address and a password. The combination of that e-mail account and password is used to ensure that only you have access to, and only you have the ability to modify, your personal information through Zero Gravity Dance's studio system. Zero Gravity Dance office personnel have access to your personal information only to manage the studio business.Zero Gravity Dance will access your personal information and/or share it with third parties only for the following purposes:
(i) as needed for studio business;
(ii) if required to do so by law or to comply with legal process; or
(iii) to protect the rights, property, or personal safety of the public.If you have any questions about our Privacy Policy or if you wish to update or remove any of your personal information, please contact the office.
Delivery: Any purchases (eg. costuming, ZGD dance gear) must be picked up at the studio front desk.
Refund: Missed classes will not be refunded. COVID-related missed classes can be accommodated: please contact front desk.
Exchange: Studio apparel may be exchanged for alternate sizes if available in studio inventory. There will be no re-ordering.
Return Policies: Costumes purchased from outside suppliers cannot be returned. Apparel, if not worn, can be returned within seven days of purchase.
Country and Currency of Transaction
All VISA, MasterCard and e-transfer transactions will be completed in Canadian (CAD) currency.
AutoPay Agreement:
Upon pressing Accept, I understand that the payment amount listed directly above (if greater than $0) will be processed immediately with the payment method/information shown above.
I also authorize Zero Gravity Dance to initiate electronic payments for the balances due on my Zero Gravity Dance account each session. Any time a payment is processed, I'll receive an e-mail notification if I have not opted out of e-mail communications. I understand that the payment amounts may vary as classes are added/dropped and as other charges/payments are applied to my account. I acknowledge that this authorization will remain in effect until I notify the office in writing that the authorization should be terminated. If for whatever reason, payments cannot be processed with the above payment information and my account balance remains overdue, I understand that my enrolment in classes will be cancelled.

One Time Pay Agreement:

I acknowledge that I am responsible to make timely payments of my balances due on my Zero Gravity Dance account. I further acknowledge that if my payment is not received on or before the due date, I authorize Zero Gravity Dance to initiate electronic payments for any balances due on my account PLUS an administrative late fee of 5% of the overdue balance. Payments will be processed with the payment method/information shown above that will be kept on-file with Zero Gravity Dance. E-mail notifications will be sent any time a payment is processed if I have not opted out of e-mail communications. I acknowledge that this authorization will remain in effect until I notify the office in writing that the authorization should be terminated. If for whatever reason, payments cannot be processed with the above payment information and my account balance remains overdue, I understand that my enrolment in classes will be cancelled.
