I hereby give my consent for Dance Company, Ltd. (DCL) to use any photos and/or videos of my minor child in DCL social media posts, on the DCL website, or in other DCL promotional or instructional materials. I understand that all such photos and videos remain the property of DCL.

I hereby acknowledge and agree to the Dance Company, Ltd (DCL) tuition rates, which can be found on the website www.dancecoltd.com, under Classes. I further agree to pay all DCL invoices within thirty (30) days of receipt. I understand that an $10 late fee will be applied to any invoice not paid within thirty (30) days of receipt. I understand that if a class is dropped after the 1st of the month, tuition payments for that month and any overdue payments are still to be paid. All invoices billed prior to the recital date must be paid in full in order to receive recital tickets.

I recognize and acknowledge that there are certain risks of physical injury to minor participants in dance instruction and performance, and I hereby agree as my child's legal guardian to assume the full risk of any and all injuries, damages, or losses, regardless of the severity, that my minor child may sustain as a result of participation in any class, instruction, or event connected with Dance Company, Ltd. (DCL), even if arising out of the negligence or fault of DCL, its owner(s), or employees (hereinafter collectively Releasees). I hereby acknowledge that I understand and am familiar with the nature of the activities in which my child will participate at DCL, and I affirm that my child is in good health and does not have any physical or emotional condition, past or present, of which I am aware, which would in any way affect my child's ability to participate in any class, instruction, or event connected with DCL.
I hereby agree to indemnify, defend, hold harmless, and fully and forever release and discharge Releasees from any and all claims, demands, causes of actions, suits, or liabilities of any kind and nature whatsoever, including attorney’s fees and court costs, relating to illness, personal injury or property damage, sustained directly or indirectly from my minor child's participation in any class, instruction, or event connected with DCL, even if caused in whole or in part by the negligence or fault of Releasees.
I further understand that it is my responsibility to have health insurance coverage in place to cover all injuries arising from participation in any class, instruction, or event connected with DCL. I also understand that I am responsible for immediately picking up my child at the end of each class or event he/she attends at DCL, and DCL employees are not responsible for the care and supervision of my minor child once class ends.
In case of emergency, I give my permission for emergency medical treatment of my child and for transportation to such treatment.

By agreeing, I attest that I have read the above document, understand it completely, and agree to the policies and waiver outlined therein.
