Starline Academy for the Performing Arts and its personnel are NOT liable for personal injuries or loss/damage of personal property. Since most lessons require physical exertion, injuries may occur. Each student is responsible for informing the instructor of any physical limitations which may prevent full participation in any class. Students and parents understand that positive and courteous behavior is required on the premises and will conduct themselves accordingly. I understand that my child chooses to withdraw from the prorgram a 30 day written cancelation is required. All fees paid prior are non-refundable.

I further understand that any pictures taken on or off the property of the students may be used for advertising and publicity (including, but not limited to: flyers, mailings, print ads, and internet) for Starline Academy.
