By signing, I give permission for the student to be acknowledged by first name or included in photos or videos on CR DanceXtreme social media or newsletters.
I acknowledge both the Christmas Spectacular in December and the Year-End Recital in June are part of the curriculum for all Recreatonal and Competitive students, aged 4 and up. My child will attend both performances.
By signing, I agree to register the student listed in the requested classes. Payment of the Registration fee and setting up an Auto-Pay account is required to guarantee a spot in the desired class. No student will be considered withdrawn from a class or the studio until the front desk is notified by email or in writing.
