By signing, I agree to register the student listed in the requested classes. Payment of the Registration fee and setting up an Auto-Pay account is required to guarantee a spot in the desired class. No student will be considered withdrawn from a class or the studio until the front desk is notified by email or in writing. Tuition Fees are Non-Refundable and Non-Transferable. By signing, I give permission for the student to be acknowledged by name or included in photos or videos on CR DanceXtreme social media or newsletters. By signing, I agree CR DanceXtreme, their instructors and employees, will not be held responsible for accident, injury or loss. I agree to release CR DanceXtreme from all claims which may be a result of accident, injury or loss.
