ACKNOWLEDGMENT OF RISK
I understand that participation in aerial silks classes involves physical movement, strength, flexibility, and elevation off the ground, which carry inherent risks of injury. These may include, but are not limited to: falls, sprains, strains, bruises, dislocations, or more serious injuries. I understand that while Peninsula Pointe Dance takes reasonable steps to provide a safe environment, risks cannot be entirely eliminated.

VOLUNTARY PARTICIPATION
I certify that I (or my child) am in good health and physically able to participate in aerial silks activities. I voluntarily choose to participate (or allow my child to participate) in these activities with full knowledge of the risks involved.

RELEASE OF LIABILITY
I agree to release and hold harmless Peninsula Pointe Dance, its owners, instructors, assistants, employees, and volunteers from any and all claims, demands, actions, or causes of action arising out of or related to any injury, damage, or loss sustained during participation in aerial silks classes or related activities, whether caused by negligence or otherwise.

MEDICAL AUTHORIZATION
In the event of an emergency, I authorize Peninsula Pointe Dance staff to secure medical treatment for me (or my child) and agree to assume full financial responsibility for such care.

AGREEMENT TO FOLLOW RULES
I agree (or my child agrees) to follow all instructions, safety rules, and guidelines provided by instructors during class. I understand that failure to do so may result in removal from the class or future participation restrictions.
