Parent/Guardian & Billing Responsibility Agreement

By creating this account and registering a student at Groove Studios, I confirm that:

1.) I am the student’s parent or legal guardian, or I have explicit permission from the student’s parent or legal guardian to register them for classes, workshops, and other activities at Groove Studios.

2.) I am the primary billing contact and agree to be financially responsible for all tuition, fees, and charges associated with the enrolled student’s participation.

3.) I understand that all communications regarding billing, payments, and account status will be directed to me.

4.) I agree to follow all Groove Studios financial policies, including payment schedules, late fees, and cancellation procedures, as outlined on the Groove Studios website (groovestudioswa.com).

By checking this box, I acknowledge that I have read, understood, and agree to the terms outlined above.

Groove Studios Participation Waiver & Release Agreement

By enrolling my child in classes, performances, workshops, and activities at Groove Studios, I do so voluntarily and affirm that my child is in appropriate physical condition to participate. If I have questions about their readiness, I will consult a healthcare provider. I will also inform the instructor or director of any known physical conditions, vulnerabilities, or injuries that may affect participation.

Release of Liability
By agreeing to this waiver, I release Groove Studios—including its owners, directors, staff, instructors, volunteers, and affiliates—from any and all liability for injury, illness, property loss, or damages resulting from my child’s participation in any Groove Studios activity, including but not limited to classes, camps, rehearsals, performances, and the use of facilities or equipment.

I understand and accept full responsibility for any risks involved, including slips, falls, sprains, strains, dislocations, and other potential injuries, even if not listed here. I voluntarily assume all risks associated with my child’s participation.

Physical Contact in Instruction
I understand that hands-on guidance is a standard part of dance education. Instructors may use appropriate physical contact to adjust posture, alignment, and movement. I agree to communicate with staff if any form of touch is not acceptable to me or my child.

Medical Treatment & Insurance
I understand that Groove Studios does not provide medical, dental, or disability insurance coverage. I authorize Groove Studios to seek emergency medical or dental treatment for my child if necessary. I agree to be financially responsible for any care provided, regardless of insurance status, and release Groove Studios from liability related to such treatment.

Photo & Video Release
I give Groove Studios permission to take photos and video recordings of my child during classes and activities. I understand these images may be used in marketing, promotions, and social media, and I waive any right to compensation or review.

Studio Policies & Jurisdiction
My child and I agree to follow all Groove Studios rules, codes of conduct, and policies. If I have concerns, I will bring them directly to the instructor or director.

I agree that any legal action related to Groove Studios participation will take place in Washington State and follow its laws. If any portion of this agreement is deemed unenforceable, the remaining terms will still apply.

By completing registration in the Groove Studios online portal, I acknowledge that I have read, understood, and agree to this Participation Waiver & Release.
