I realize signing up for in-studio dance classes that there is a risk of injury. In the event of an emergency, I authorize Rhythm Dance Center to use reasonable discretion, in rendering first aid and/or arranging for emergency medical care (including hospitalization), at my expense. In the event of an accident or injury during this program or any other organized event that results from this program, I agree to waive any and all liability to Rhythm Dance Center and its instructors.
