I agree that I am allowing my child to participate in Tempo Dance Studio LLC's class programs. I agree that my child has full approval from a physician to participate in physical fitness activities and that my child is fully capable of participating fully in a physical fitness or dance style class to the best of my knowledge. I agree to assume full responsibility for any injury, risk, or damage whether known or unknown which might incur as part of participating in the program. I knowingly, voluntarily and expressly release any claim I may have against Tempo Dance Studio LLC or its instructors or affiliates. I, my heirs, or legal representatives forever release, waive, discharge and covenant not to sue Tempo Dance Studio LLC, its' instructors or affiliates for any injury or death caused by their negligence or other acts.

I hereby grant Tempo Dance Studio and its respective licensees, successors, affiliates, and assigns permission to use and reproduce any images of my child (children) in any format whatsoever for promotional purposes including but not limited to marketing, advertising publicity, print materials, social media, websites, and internet advertising. Said images are the property of Tempo Dance Studio, LLC and no compensation whatsoever is paid for the use thereof. I agree to fully indemnify, hold harmless and release all claims against Tempo Dance Studio, LLC in relation to said name and images.

I have read the above release of waiver and liability and fully understand its contents. I voluntarily agree to the terms and conditions above.
