As a student of Studio D Dance, your child or dancer is expected to abide by the following code of conduct:

1. Respect: They will treat all instructors, staff, fellow dancers, and studio visitors with respect at all times.

2. Attendance and Punctuality: They will arrive on time for all dance classes and rehearsals. If they are unable to attend, they will notify the studio in advance.

3. Dress Code: They will wear appropriate dance attire and footwear as instructed by their instructors.

4. Behavior: They will listen to their instructors and follow their directions during class. They will refrain from disruptive behavior, use of inappropriate language, or engaging in any form of bullying or teasing.

5. Participating in our end-of-class showcase is a special opportunity for students to shine and celebrate their progress in ballet. It's a chance for them to proudly display all they've learned and accomplished throughout their time in class. We want every student to have the chance to showcase their hard work and dedication.

However, it's important to remember that showcase participation is a privilege that comes with consistent attendance and commitment. If a student misses excessive classes without notice or explanation, they may not be able to participate in the showcase.

By accepting these terms, you acknowledge that you have read and understand the Studio D Dance Code of Conduct Agreement for Students and agree to ensure that your child or dancer adheres to its terms and guidelines. At Studio D, we prioritize creating a safe and supportive environment where every dancer can thrive and enjoy their dance experience. In the event that a child violates the behavior code outlined in the agreement, Studio D reserves the right to deny services to them. This policy is in place to protect the well-being of all individuals and uphold the positive atmosphere of our studio, ensuring that each dancer's experience is valued and respected.

I, hereby authorize Studio D Dance to seek and obtain medical treatment on my behalf in the event of an injury or medical emergency that may occur during my participation in dance classes, rehearsals, performances, or any other studio-related activities.

I understand and acknowledge that Studio D Dance, its owners, instructors, employees, and agents will make every effort to ensure my safety during studio activities. However, in the event of an injury or medical emergency, I authorize Studio D Dance to take appropriate action, including but not limited to, calling emergency medical services, transporting me to a medical facility, and administering first aid.

I further authorize Studio D Dance to disclose relevant medical information about dancer to medical professionals and emergency responders for the purpose of providing medical treatment.

I acknowledge that I am at least 18 years of age and am freely providing this medical release. If I am under 18 years of age, my parent or legal guardian has read and consented to the terms of this medical release form on my behalf.

All fees are non-refundable. Tuition is due on the 1st of each month. A late fee charge a 25.00 fee 3 days after the due date. If left unpaid, another fee of 10.00 is charged every 7 days. Tuition will not be individually prorated for any circumstance. Full tuition is due each month regardless of the holidays. Full tuition is due each month whether your child takes a class that month or not. No exceptions.
In the event that your child's tuition account does not remain in good standing, your child will not be able to participate in dance class or end-of-season showcase. If your child has a showcase costume fee it is due on due date stated in your transaction journal. Late fees will apply.
Autopay is required to register. We require ALL parents to have a valid credit card on file for automatic payments.
Registration fees are non-refundable. All monthly tuition payments, fees, will be auto-drafted to the card on file. You will be emailed a receipt after your tuition is auto-drafted. You will not receive a billing statement or invoice prior to any charges, however you will receive a receipt of payment each month after the auto-draft.
No cash or checks are accepted.

Studio D Dance Liability Waiver and Release

In consideration of my child's participation in activities at Studio D Dance, I hereby release and covenant not to sue Studio D Dance, its owners, staff, agents, volunteers, and members from any and all present and future claims resulting from ordinary negligence on the part of Studio D Dance or others listed, for property damage, personal injury, or wrongful death arising from my child's engagement in or receipt of instruction in dance, tumbling, movement, cheerleading, or any other related activities, wherever, whenever, or however the same may occur. I hereby voluntarily waive any and all claims resulting from ordinary negligence, both present and future, that may be made by me, my family, estate, heirs, or assigns.

I am aware that dancing and aerobic activities are vigorous and involve lifts and rotations in a unique environment, which pose inherent risks of injury. I understand that dance, tumbling, movement, cheerleading, and related activities always involve certain risks, including but not limited to, death, serious neck injury, spinal injuries resulting in complete or partial paralysis, brain damage, and serious injury to virtually all bones, joints, muscles, and internal organs. I acknowledge that the safety equipment and apparatus provided, including the active participation of instructors who may spot and assist in performing certain skills, may not fully prevent serious injury. I understand that participation in these activities, including movement from event to event, conditioning, stretching, and other activities, may leave my child vulnerable to the actions of other participants who may not have full control of their actions or may not see others on the floor. I am voluntarily allowing my child to participate in these activities with full knowledge of the risks involved and agree to accept any and all inherent risks of property damage, personal injury, or death.

I further agree to indemnify and hold harmless Studio D Dance and all others listed for any and all claims arising as a result of my child’s engagement in or receipt of instruction at Studio D Dance, including any activities incidental thereto, whenever, wherever, or however the same may occur.

I understand that this waiver is intended to be as broad and inclusive as permitted by the laws of the State of Georgia and agree that if any portion is held invalid, the remainder of the waiver will continue in full legal force and effect. I further agree that the venue for any legal proceedings will be held within the State of Georgia.

I affirm that I am of legal age and am freely accepting this agreement. I have read this form and fully understand that by accepting this form, I am giving up legal rights and/or remedies which may be available to me for the ordinary negligence of Studio D Dance or any person listed above.

By accepting this waiver and release of liability, I hereby represent that I am the parent or legal guardian of the child who will participate in the activities or receive instruction from Studio D Dance. I am authorized to execute this document on behalf of my child. Furthermore, I represent that consent or authorization from any other person is not legally required.

Studio D Dance Withdrawal Policy

This policy outlines the procedure and requirements for withdrawing a child from classes at Studio D Dance.

To withdraw your child from classes, a 30-day written notice is required. The notice must be submitted via email to info@danceatstudiod.com.

The notice should be sent before the start of the next billing cycle to avoid additional charges. If a 30-day written notice is not provided, you will be charged for the following month of classes. This charge will be applied to cover administrative and scheduling costs.
