By creating an account, you confirm that you are the parent or legal guardian of the child(ren) being registered, and that you are at least 18 years of age. You agree to abide by Studio D Dance's policies, including our Payment Policy, Code of Conduct, and Withdrawal Policy, as outlined during enrollment.

As a student of Studio D Dance, your child or dancer is expected to abide by the following code of conduct:

1. Respect
They will treat all instructors, staff, fellow dancers, and studio visitors with respect at all times.

2. Attendance and Punctuality
They will arrive on time for all dance classes and rehearsals. If they are unable to attend, a parent/guardian will notify the studio in advance.

3. Dress Code
Students are required to wear their studio-provided dancewear (leotard, tights, and hair accessories) to every class. Hair must be tied back and secured away from the face in a ponytail, bun, or similar secure hairstyle — no loose hair during class. Ballet shoes must be worn to every class, as proper footwear plays an important role in each dancer's foot and skill development. Students who arrive without proper attire or footwear may be asked to observe rather than participate for that class.

4. Behavior
They will listen to their instructors and follow directions during class. They will refrain from disruptive behavior, use of inappropriate language, or engaging in any form of bullying or teasing.

5. Recital Participation
Participating in our end-of-season recital is a special opportunity for students to shine and celebrate their progress in ballet — a chance to proudly display all they've learned and accomplished throughout the season. We want every student to have the opportunity to showcase their hard work and dedication.

However, recital participation is a privilege that comes with consistent attendance, commitment, and timely enrollment. Students enrolling after April 14 (one month prior to recital) may continue regular classes but are not guaranteed placement in the recital performance, as adequate preparation time is required. Likewise, students who miss excessive classes without notice or explanation may be excused from the recital lineup at the studio's discretion, based on attendance and readiness.

By accepting these terms, you acknowledge that you have read and understand the Studio D Dance Code of Conduct Agreement for Students and agree to ensure that your child or dancer adheres to its terms and guidelines. At Studio D, we prioritize creating a safe and supportive environment where every dancer can thrive and enjoy their dance experience. In the event that a child violates the behavior code outlined in this agreement, Studio D reserves the right to deny services to them. This policy is in place to protect the well-being of all individuals and uphold the positive atmosphere of our studio, ensuring that each dancer's experience is valued and respected.

As the parent or legal guardian of the above-named dancer, I hereby authorize Studio D Dance to seek and obtain medical treatment on my child's behalf in the event of an injury or medical emergency that may occur during participation in dance classes, rehearsals, performances, or any other studio-related activities.

I understand and acknowledge that Studio D Dance, its owners, instructors, employees, and agents will make every effort to ensure my child's safety during studio activities. However, in the event of an injury or medical emergency, I authorize Studio D Dance to take appropriate action, including but not limited to calling emergency medical services, transporting my child to a medical facility, and administering first aid.

I further authorize Studio D Dance to disclose relevant medical information about my child to medical professionals and emergency responders for the purpose of providing medical treatment.

I certify that any medical information provided to Studio D Dance (allergies, conditions, medications, etc.) is accurate and complete to the best of my knowledge, and I agree to notify Studio D Dance promptly of any changes.

All fees are non-refundable, including registration, dancewear, costume, and recital fees.

Tuition
Monthly tuition is $45 (ages 3–5) or $55 (under 3), due on the 1st of each month via autopay. Full tuition is due each month regardless of holidays or planned studio closures, and whether your child attends class that month or not — no exceptions. Tuition is not prorated mid-month for missed classes, but new students joining mid-season will have their first month's tuition prorated based on actual start date.

Late Tuition Fee
A $20 late fee applies if tuition is not received within 5 days of the due date (starting the 6th of the month).

Account Standing & Class Access
If a student's account becomes more than 10 days past due, the student may be withheld from class until the account is brought current. Withdrawal from the program does not waive any tuition, fees, or balance owed for classes already attended. See the Withdrawal Policy for full notice requirements.

Autopay Requirement
Autopay is required to register. All parents must have a valid credit card on file. No cash or checks are accepted.

Registration Fee
A registration fee of $30 (ages 3–5) or $45 (under 3) is due at the time of enrollment. Families enrolling within the first two weeks of the season (by August 28) pay the standard registration fee only. Students joining after this window will also be assessed a late enrollment fee — $15 for enrollments through September 25, and $30 for enrollments after that date. This does not apply to students becoming age-eligible mid-season.

Dancewear Fee
A one-time $40 dancewear fee is charged at enrollment and includes leotard, tights, ballet shoes, and hair accessories for the semester.

Costume Fee
A $45 costume fee will be charged on February 15.

Recital Fee
A $20 recital fee will be charged on April 15.

Recital Eligibility & Refunds
If your child's account does not remain in good standing, your child may not be able to participate in dance class or the end-of-season recital. Students enrolling after April 14 (one month prior to recital) may continue regular classes but are not guaranteed recital placement. Costume and recital fees are non-refundable, including in the event a student is excused from recital participation due to attendance or eligibility. The costume remains the student's to keep.

Fee Authorization Agreement
By signing below, I authorize Studio D Dance to automatically charge the card on file for the following: Registration Fee (at enrollment), Dancewear Fee (at enrollment), Costume Fee (February 15), Recital Fee (April 15), and Monthly Tuition (1st of each month, ongoing). I understand that late fees and withdrawal fees may apply as outlined above if any charge is not successfully processed or if proper notice is not given.

Studio D Dance Liability Waiver and Release

In consideration of my child's participation in activities at Studio D Dance, I hereby release and covenant not to sue Studio D Dance, its owners, staff, agents, volunteers, and members from any and all present and future claims resulting from ordinary negligence on the part of Studio D Dance or others listed, for property damage, personal injury, or wrongful death arising from my child's engagement in or receipt of instruction in dance, tumbling, movement, cheerleading, or any other related activities, wherever, whenever, or however the same may occur. I hereby voluntarily waive any and all claims resulting from ordinary negligence, both present and future, that may be made by me, my family, estate, heirs, or assigns.

I am aware that dancing and aerobic activities are vigorous and involve lifts and rotations in a unique environment, which pose inherent risks of injury. I understand that dance, tumbling, movement, cheerleading, and related activities always involve certain risks, including but not limited to, death, serious neck injury, spinal injuries resulting in complete or partial paralysis, brain damage, and serious injury to virtually all bones, joints, muscles, and internal organs. I acknowledge that the safety equipment and apparatus provided, including the active participation of instructors who may spot and assist in performing certain skills, may not fully prevent serious injury. I understand that participation in these activities, including movement from event to event, conditioning, stretching, and other activities, may leave my child vulnerable to the actions of other participants who may not have full control of their actions or may not see others on the floor. understand that Studio D Dance's Studio Program may take place at school facilities or other locations outside of a traditional studio setting, and the risks described above apply equally in these environments. I am voluntarily allowing my child to participate in these activities with full knowledge of the risks involved and agree to accept any and all inherent risks of property damage, personal injury, or death.

I further agree to indemnify and hold harmless Studio D Dance and all others listed for any and all claims arising as a result of my child’s engagement in or receipt of instruction at Studio D Dance, including any activities incidental thereto, whenever, wherever, or however the same may occur.

I understand that this waiver is intended to be as broad and inclusive as permitted by the laws of the State of Georgia and agree that if any portion is held invalid, the remainder of the waiver will continue in full legal force and effect. I further agree that the venue for any legal proceedings will be held within the State of Georgia.

I affirm that I am of legal age and am freely accepting this agreement. I have read this form and fully understand that by accepting this form, I am giving up legal rights and/or remedies which may be available to me for the ordinary negligence of Studio D Dance or any person listed above.

By accepting this waiver and release of liability, I hereby represent that I am the parent or legal guardian of the child who will participate in the activities or receive instruction from Studio D Dance. I am authorized to execute this document on behalf of my child. Furthermore, I represent that consent or authorization from any other person is not legally required.

This policy outlines the procedure and requirements for withdrawing a child from classes at Studio D Dance.

To withdraw your child from classes, 30 days' written notice is required. The notice must be submitted via email to info@danceatstudiod.com.

The notice should be sent before the start of the next billing cycle to avoid additional charges. If 30 days' written notice is not provided, you will be charged for the following month of classes. This charge covers administrative and scheduling costs, and does not waive any tuition, fees, or balance owed for classes already attended.
