Southern Ballet Theatre Parent/Guardian Agreement

By creating an account and enrolling my child at Southern Ballet Theatre, I acknowledge and agree to the following:

Assumption of Risk: I understand that dance training involves physical activity and inherent risks. I release Southern Ballet Theatre, its directors, and teaching staff from liability for any injuries sustained during classes, rehearsals, or performances.

Medical Emergencies: In the event of a medical emergency where I cannot be reached, I authorize Southern Ballet Theatre staff to seek necessary medical treatment for my child.

Fee Policy: I agree to pay all term fees and associated costs by the specified due dates. I understand that term fees are generally non-refundable once the term has commenced.

Code of Conduct & Uniform: I agree to support the Southern Ballet Theatre’s Code of Conduct. I will ensure my child attends classes punctually, treats staff and peers with respect, and adheres to the required uniform and grooming standards.

Media & Photography: I have reviewed the theatre's policy regarding photography and videography, and I understand the guidelines surrounding the use of my child's image for Southern Ballet Theatre's promotional or archival purposes.

I confirm that I am the legal parent or guardian of the registered student(s), I am over 18 years of age, and I agree to abide by all current Southern Ballet Theatre policies.

I hereby acknowledge that I am voluntarily enrolling in classes offered by Southern Ballet Theatre ("the Organization"). In consideration for being permitted to participate in these classes and activities, I hereby agree to the following terms and conditions:

Payment Acknowledgment: I acknowledge that I am responsible for the payment of tuition fees for the classes in which I am enrolling.

Assumption of Risk: I understand and acknowledge that participation in dance classes and related activities carries with it certain inherent risks, including but not limited to physical injury. I voluntarily assume all risks associated with my participation in these activities.

Release of Liability: I hereby release, waive, discharge, and covenant not to sue the Organization, its directors, officers, employees, agents, and volunteers from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, or to any property belonging to me, while participating in these activities.

Indemnification: I agree to indemnify and hold harmless the Organization, its directors, officers, employees, agents, and volunteers from any loss, liability, damage, or costs, including court costs and attorney fees, that they may incur due to my participation in these activities.

Refund Policy: I understand and agree to abide by the Organization's refund policy regarding tuition fees, as outlined by Southern Ballet Theatre.

Southern Ballet Theatre
I/we realize that participation in dance classes and activities could involve some possible personal injury. Despite precautions, accidents and injuries may occur. By signing this release form, I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces used by Southern Ballet Theatre.
I/we agree to release and hold harmless Southern Ballet Theatre including its teachers, dancers, staff members and facilities used by Southern Ballet Theatre from any cause of action, claims, or demands now and in the future. I/we will not hold Southern Ballet Theatre liable for any personal injury or any personal property damage, which may occur on the premises before, during or after classes. Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities utilized by Southern Ballet Theatre.
I understand that Southern Ballet Theatre is licensed, accredited and insured. In the event that I/we should observe any unsafe conduct or conditions before, during or after my/our classes, I/we agree to report the unsafe conduct or conditions to the General Manager, Artistic Director, Board of Trustees, Individual Instructor or staff member as soon as possible.

Dear Parent/Guardian,

At Southern Ballet Theatre, we occasionally take photographs and record videos of our classes, rehearsals, and performances to showcase the exciting journey and achievements of our students. These images and footage may be used in our brochures, posters, newsletters, social media, website, and other promotional materials.

We adhere to New Zealand privacy laws and best practices in our handling and use of your child's photographs and videos. We kindly request your permission to use photographs and videos that include your child for the purposes outlined above.

By accepting this waiver I grant Southern Ballet Theatre permission to take and use photographs and videos of my child as described above. I understand that this content may appear in public and various media formats associated with Southern Ballet Theatre.
Additional Permissions:
I agree that Southern Ballet Theatre may use these images and videos beyond the current academic year.
I consent to Southern Ballet Theatre sharing these images and videos with media organizations for related publicity.
I understand that I can revoke my consent at any time by writing to Southern Ballet Theatre at the address provided above, though I understand that any materials already published prior to revocation of my consent will continue to be in circulation.
