I understand that risk of injury is inherent in any physical activity and I, on behalf of the myself/my child, knowingly and voluntarily accept that risk. I, the undersigned, hereby waive and release Sweta Ravisankar or any other teachers teaching at Sarada Kala Nilayam individually and Sarada Kala Nilayam from any and all claims or damages of any kind arising out of my or my child’s participation in the
exercise and/or dance program at Sarada Kala Nilayam. I further certify that the aforementioned student is in proper physical condition to participate in the exercise/dance.
In addition, participation, attendance, and/or engagement at Sarada Kala Nilayam include possible exposure to an illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death do exist; and, I knowingly agree to assume all risks, both known and unknown, and assume full responsibility for my/my child's participation; and, I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, if I observe any unusual or significant hazard during my/my child's presence or participation, I will remove myself /my child from participation and bring such information to the attention of the management immediately.
