Media Release Policy
I understand that Boyd Dance Studios may photograph and/or video record me and/or my child during classes, rehearsals, performances, and studio events for educational and promotional purposes.
I give permission for Boyd Dance Studios, its staff, and representatives to use these photos and videos in print and digital formats, including but not limited to the studio website, social media, local newspapers, and other marketing or advertising materials.
I understand that names will not be published without additional consent.
This authorization is voluntary and may be withdrawn in writing at any time.

Minor Injury / First Aid Policy
In the event of a minor injury, I authorize Boyd Dance Studios and its staff to administer basic first aid as deemed reasonably necessary.
I understand that every effort will be made to notify a parent or guardian in the event of an injury.
This authorization does not include medical treatment beyond basic first aid.

Payment Policy
I understand that tuition is billed monthly and is due in advance on the 1st of each month.
Monthly tuition remains the same regardless of the number of weeks in a given month and is not adjusted for absences, vacations, holidays, illness, or weather-related closures. Tuition is non-refundable and non-transferable.
Make-up classes are encouraged for missed classes and must be completed within the same month as the absence.
Accounts not paid by the 1st of the month are considered past due and will incur a $15.00 late fee.
I understand that written notice of withdrawal must be submitted prior to the 15th of the month to avoid being charged for the following month’s tuition.

Liability Waiver & Assumption of Risk
I understand that participation in dance, tumbling, and related activities at Boyd Dance Studios involves inherent risks, including but not limited to physical injury such as strains, sprains, or other injuries. I also acknowledge the potential risk of exposure to communicable illnesses.
By enrolling, I voluntarily assume all risks associated with participation for myself and/or my child.
I hereby release, waive, and hold harmless Boyd Dance Studios, its owners, staff, instructors, and representatives from any and all liability, claims, demands, or causes of action arising out of or related to participation in studio activities, whether occurring on or off the premises.
I acknowledge that participation is voluntary and agree that Boyd Dance Studios shall not be held liable for any injuries or damages, except where prohibited by law.
