REGISTRATION AND PAYMENT TERMS a) By completing this registration, you agree to enroll in MIM Empowered's programs and comply with all policies and procedures. b) Current program fees will be provided upon registration and are subject to change. c) Additional fees may include: - Costume fees - Team jacket - Optional makeup kit - Competition fees (if applicable) d) Payment is required at the time of registration to secure your spot. e) Late Payment Policy: - A 10% late fee will be applied every 7 days on outstanding balances - Late fees begin accumulating from the payment due date - Multiple late fees may be applied if payment remains outstanding 2. COMPETITION AND PERFORMANCE PARTICIPATION a) Competition and performance fees will be provided separately upon registration b) All competition-related fees must be paid in full before participation 3. CLASS POLICIES a) Student Placement: - Students will be placed in appropriate levels based on age, skill, and aptitude - Class ratios are maintained at: * Ages 5 and under: 5:1 * Ages 6-12: 10:1 * Ages 13+: 15:1 4. CANCELLATION AND REFUND POLICY a) No refunds will be issued after the second class for group lessons b) Private lessons require 2 weeks' notice for cancellation c) Make-up classes are only offered when 2 weeks' notice is provided for absences 5. PERFORMANCE AND SHOW PARTICIPATION a) Students may participate in up to 2 recitals and 4 musical theatre performances annually b) Additional show fees and costume requirements will be communicated in advance c) Show participation is encouraged but not mandatory 6. STUDIO COMMUNICATIONS a) Official communications will be sent through Class Dojo and email b) Parents/guardians are responsible for maintaining updated contact information c) Regular updates about student progress will be provided through progress cards and midterm evaluations 7. HEALTH AND SAFETY a) Parents/guardians must disclose any relevant medical conditions or allergies b) Students must adhere to all studio safety protocols and guidelines c) MIM Empowered reserves the right to modify class structures for safety reasons 8. PHOTO AND VIDEO RELEASE a) MIM Empowered may photograph or video record students during classes, rehearsals, and performances b) These materials may be used for promotional purposes, including social media and marketing materials c) Parents may opt out of photo/video usage by submitting a written request 9. CODE OF CONDUCT a) Students and parents must adhere to MIM Empowered's anti-bullying policy b) Respectful behavior towards staff and fellow students is required at all times c) Violation of conduct policies may result in dismissal from the program 10. SCHEDULE AND CALENDAR a) MIM Empowered follows a standard academic year calendar (September-June) b) Summer programs are offered separately and require additional registration c) The studio reserves the right to modify schedules as needed 11. LIABILITY WAIVER a) Participants acknowledge that dance, music, and theatre activities involve inherent risks b) MIM Empowered is not liable for personal injuries or lost/stolen property c) Parents/guardians assume all risks associated with participation 12. AGREEMENT ACKNOWLEDGMENT By completing this registration, you acknowledge that you have read, understood, and agree to all terms and conditions outlined in this agreement. You also confirm that all information provided during registration is accurate and complete.
