Dance Program Parent/Guardian Waiver & Policies Agreement
2026–2027 Dance Season
By enrolling my child in the Soul Studios, LLC Dance Program, I acknowledge that I am the parent or legal guardian of the participating minor and have the authority to enter into this agreement on their behalf. I understand and agree to the following terms, policies, and conditions of participation.
1. ASSUMPTION OF RISK
I understand that participation in dance, tumbling, acro, conditioning, rehearsals, performances, and other activities offered through Soul Studios involves inherent risks. These risks may include, but are not limited to, falls, collisions, muscle strains, sprains, fractures, illness, physical exertion, and other injuries.
I voluntarily allow my child to participate in these activities and knowingly assume the risks associated with their participation.
I understand that instructors will make reasonable efforts to provide appropriate instruction and maintain a safe environment; however, injuries may occur even when reasonable safety precautions are followed.
2. RELEASE OF LIABILITY
To the fullest extent permitted by applicable law, I release and hold harmless Soul Studios, LLC, its owners, employees, instructors, contractors, volunteers, agents, and representatives from claims arising from the ordinary and inherent risks associated with my child's participation in Soul Studios activities.
I understand that this agreement is intended to provide Soul Studios with the maximum protection permitted by applicable law and does not waive rights or claims that cannot legally be waived.
3. MEDICAL RESPONSIBILITY & EMERGENCY AUTHORIZATION
I understand that it is my responsibility to determine whether my child is physically able to safely participate in dance and related activities. I am responsible for informing Soul Studios of any relevant limitations, injuries, allergies, or other information necessary for the safe participation of my child.
I understand that Soul Studios instructors and staff are not responsible for diagnosing or treating medical conditions.
In the event of an injury, illness, or emergency, I authorize Soul Studios staff to provide reasonable first-aid assistance and to contact emergency medical services when they believe it is necessary.
Soul Studios reserves the right to call 911 or seek emergency medical assistance for my child when staff reasonably believe emergency care is warranted, even if a parent or guardian cannot first be reached.
I understand that I am financially responsible for any medical care, ambulance transportation, emergency services, or other medical expenses incurred on behalf of my child.
4. PARENT/GUARDIAN CONTACT INFORMATION
I agree to maintain current contact and emergency contact information with Soul Studios throughout the dance season.
I understand that I am responsible for being reasonably reachable during my child's classes, rehearsals, performances, and studio events when required.
5. MONTHLY TUITION & PAYMENT
I understand that dance tuition is billed monthly according to Soul Studios' current tuition schedule.
Tuition reserves my child's placement in their class and is not based on the number of classes attended during a particular month.
Tuition is not prorated or refunded for absences, vacations, illness, holidays, scheduled studio closures, or other missed classes unless otherwise specifically determined by Soul Studios.
I understand that tuition and other applicable fees are non-refundable once charged unless Soul Studios determines otherwise.
I authorize Soul Studios to charge the payment method on file for tuition and other agreed-upon dance program fees according to the applicable payment schedule.
6. CANCELLATION & WITHDRAWAL
I understand that enrollment in the Soul Studios Dance Program is an ongoing commitment unless proper withdrawal procedures are completed.
A written cancellation request must be submitted at least 30 days before withdrawal from the dance program.
I understand that tuition payments scheduled within the required 30-day notice period remain due and will be charged to the payment method on file.
Simply discontinuing attendance does not constitute cancellation.
I understand that I remain financially responsible for tuition until the required cancellation process has been completed.
7. ATTENDANCE & CLASS PARTICIPATION
Consistent attendance is important for my child's individual progress and for the progress of their entire class.
I understand that choreography, technique, formations, partnering, and performance preparation depend on dancers attending class consistently.
If my child must miss class, I understand that they may be responsible for learning missed choreography or material.
Soul Studios cannot guarantee make-up classes for absences.
Excessive absences, particularly during recital preparation, may affect my child's placement in choreography or their ability to participate in portions of a performance if instructors determine they are not adequately prepared to perform safely and confidently.
8. RECITAL & PERFORMANCE EXPECTATIONS
I understand that participation in the annual recital requires additional preparation outside of normal weekly instruction.
Recital-related costs may include costumes, recital fees, photographs, apparel, tickets, or other expenses communicated separately by Soul Studios.
I understand that recital fees and deadlines will be communicated in advance and that it is my responsibility to review and meet those deadlines.
I understand that recital costumes, choreography, formations, music, schedules, and performance assignments are determined by Soul Studios and its dance staff.
I understand that recital participation requires attendance at required rehearsals, blocking, dress rehearsals, and performances as communicated by Soul Studios.
Failure to attend a required rehearsal may affect my child's ability to perform if Soul Studios determines that participation would be unsafe or disruptive to the class's performance.
9. RECITAL FUNDRAISING
I understand that any parent-led fundraising opportunities offered to help offset individual family recital expenses are separate from Soul Studios, LLC.
Soul Studios, LLC is not conducting, managing, or guaranteeing these fundraising activities.
When fundraising opportunities are organized through a parent organization or PTO, funds will be managed by that organization according to its own procedures and may be applied toward eligible recital expenses for participating families.
Participation in fundraising is optional and does not guarantee that all recital expenses will be covered.
10. DRESS CODE & CLASS PREPARATION
I understand that my child is expected to arrive prepared for class and follow the dress code established for their class.
This may include appropriate dancewear, tights, shoes, hair requirements, and other class-specific items.
Proper attire and footwear are important for safety, instruction, and the instructor's ability to observe technique and alignment.
11. BEHAVIOR & CODE OF CONDUCT
Soul Studios strives to maintain a safe, respectful, encouraging, and positive environment for all dancers, families, instructors, and staff.
I agree that my child and our family will treat instructors, staff, other dancers, and other families respectfully.
Bullying, harassment, threatening behavior, inappropriate language, unsafe conduct, repeated disruption, or behavior that negatively affects the studio environment will not be tolerated.
Soul Studios reserves the right to remove a dancer or family from a class, event, performance, or the dance program when behavior is unsafe, disruptive, threatening, or inconsistent with studio expectations.
Depending on the circumstances, removal from the program may occur without refund.
12. INSTRUCTOR & CLASS CHANGES
I understand that Soul Studios may make reasonable changes to instructors, class schedules, class levels, class locations, choreography, or class assignments when necessary.
Soul Studios reserves the right to move a dancer to a different class or level when the dance staff determines another placement would better support the dancer's development or the needs of the program.
13. STUDIO CLOSURES & SCHEDULE CHANGES
I understand that Soul Studios maintains an annual studio calendar that includes scheduled closures, holidays, recital activities, and other important dates.
Soul Studios may also cancel or modify classes because of weather, instructor illness, emergencies, facility issues, low attendance, or other circumstances outside of normal scheduling.
Tuition is calculated as part of the overall dance season and is not prorated based on scheduled studio closures.
14. STUDIO PRO PARENT PORTAL & COMMUNICATION
I understand that the Studio Pro Parent Portal/App is an essential communication tool for the Soul Studios Dance Program.
Parents/guardians are responsible for maintaining access to the app and ensuring that their contact information is current.
Important information including the studio calendar, announcements, recital information, schedule changes, account information, and other dance communications may be distributed through the Parent Portal/App and/or email.
I understand that it is my responsibility to regularly review studio communications and stay informed regarding my child's dance program.
Failure to read studio communications does not remove responsibility for communicated deadlines, schedules, fees, or requirements.
15. DROP-OFF, PICK-UP & SUPERVISION
I understand that Soul Studios is responsible for supervising my child only during their scheduled class, rehearsal, or designated studio activity.
I am responsible for arranging timely drop-off and pick-up.
Children should not be left at the studio significantly before their scheduled class or remain unsupervised after their class has ended unless they are participating in another scheduled Soul Studios activity.
Once my child has been released to me or another authorized adult, Soul Studios is no longer responsible for their supervision.
16. PERSONAL PROPERTY
I understand that Soul Studios is not responsible for lost, stolen, misplaced, or damaged personal belongings.
Families are encouraged to clearly label dance shoes, clothing, bags, water bottles, and other personal items.
17. PHOTO & MEDIA RELEASE
I grant Soul Studios, LLC permission to photograph or record my child during classes, rehearsals, performances, studio events, and other Soul Studios activities.
I authorize Soul Studios to use these photographs or recordings for legitimate studio promotional and marketing purposes, including social media, the Soul Studios website, printed materials, advertisements, and promotional content, without compensation.
If I do not consent to the use of my child's image for promotional purposes, I understand that I must notify Soul Studios in writing.
Soul Studios will make reasonable efforts to honor written media restrictions.
18. SECURITY CAMERAS
I understand that Soul Studios may utilize security cameras within portions of its facilities for safety, security, training, and operational purposes.
These systems may record video and/or audio where legally permitted.
Cameras will not be intentionally placed in private areas where there is a reasonable expectation of privacy.
19. ILLNESS & INJURY
I agree not to send my child to class when they are experiencing a contagious illness or when participation may place themselves or others at unnecessary risk.
If my child becomes ill or injured during class and staff determine they should no longer participate, I understand that I may be contacted and required to pick them up.
Soul Studios and its instructors reserve the right to modify or restrict a dancer's participation when they reasonably believe continued participation may be unsafe.
20. RIGHT TO REFUSE OR TERMINATE PARTICIPATION
Soul Studios reserves the right to refuse participation, suspend participation, or terminate enrollment when reasonably necessary to protect the safety, operations, staff, dancers, or overall environment of the studio.
This may include, but is not limited to, unsafe behavior, harassment, repeated violations of studio policies, significant disruption, nonpayment, or conduct that interferes with Soul Studios' ability to safely and effectively operate its program.
21. PARENT/GUARDIAN ACKNOWLEDGMENT
By signing this agreement, I acknowledge that:
I am the parent or legal guardian of the participating minor and have authority to sign on their behalf.
I have read and understand this entire agreement.
I understand the inherent risks associated with dance and related physical activities.
I understand Soul Studios' tuition, cancellation, attendance, recital, communication, conduct, safety, and program policies.
I accept financial responsibility for the tuition and applicable fees associated with my child's enrollment.
I agree to follow Soul Studios policies and ensure that my child follows applicable studio expectations.
I understand that studio policies may be reasonably updated when necessary and that significant changes will be communicated to families.
I voluntarily consent to my child's participation in the Soul Studios Dance Program.
By signing below, I acknowledge that I have had the opportunity to review this agreement and agree to its terms.
