Soul Studios Waiver and Release Form
I, hereby agree to the following terms and conditions for participation in classes and activities at Soul Studios:
Photo Release: I grant Soul Studios permission to use photographs and/or videos taken of me during classes or events for promotional purposes, including but not limited to social media, website, and marketing materials.
Refunds and Credits: I understand that refunds for plans or packages will be issued in the form of credits to my account at Soul Studios.
Package Pause: If I request a pause on my package and the pause releases at the time I requested, I understand that no refund will be processed for the paused period. I acknowledge that it is my responsibility to remember the unpause date.
Assumption of Risk: I understand and acknowledge that participation in fitness classes and activities at Soul Studios carries with it inherent risks of physical injury. I voluntarily assume all risks associated with such participation, including but not limited to falls, contact with other participants, and any effects of the physical activity.
Release of Liability: I hereby release, discharge, and hold harmless Soul Studios, its owners, instructors, employees, and agents from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, or to any property belonging to me, while participating in classes or activities at Soul Studios.
Lost or Stolen Items: I understand that Soul Studios is not responsible for any lost or stolen items from the studio premises. Soul Studios clears its lost and found once a month, and I agree not to leave my belongings at the studio unless I have purchased a locker subscription.
I have read this waiver and release form, fully understand its terms, and voluntarily agree to be bound by them.

Liability Release Waiver
I, hereby acknowledge that I have voluntarily enrolled myself/ my child, in the fitness and dance program offered by Soul Studio, LLC. In consideration for being allowed to participate in the program, I agree to the terms and conditions set forth in this Liability Release Waiver.
Assumption of Risk: I understand that participation in any fitness and dance program involves certain inherent risks, including, but not limited to, the risk of physical injury. I fully understand and acknowledge these risks and accept full responsibility for any injury, loss, or damage that may occur during the course of my participation or my child's participation in the program.
Release of Liability: I hereby release and discharge Soul Studio, LLC its owners, instructors, employees, volunteers, and any associated parties, from any and all claims, liabilities, actions, damages, costs, or expenses that may arise as a result of my participation or my child's participation in the fitness and dance program. This includes, but is not limited to, any claims for personal injury, property damage, or wrongful death.
Medical Authorization: I authorize Soul Studios, LLC and its representatives to seek medical treatment for myself or my child in the event of an emergency. I understand that efforts will be made to contact me or the emergency contact listed in my registration form prior to any medical treatment being administered. However, if I cannot be reached, I consent to the necessary medical treatment being given to myself or my child.
Code of Conduct: I agree to abide by all the rules, regulations, and instructions set forth by Soul Studio, LLC and its instructors during my participation or my child's participation in the fitness and dance program. I understand that failure to comply with these rules may result in my removal or my child's removal from the program without refund.
Photographic Release: I grant Soul Studio, LLC permission to use photographs or videos taken during the program for promotional and marketing purposes, including website, social media, and advertising materials.
I have read this Liability Release Waiver carefully and fully understand its contents. I am aware that it includes a release of liability and I sign it voluntarily as my own free act. By signing below, I acknowledge that I am waiving certain legal rights, including the right to sue Soul Studio, LLC, its owners, instructors, employees, and volunteers.1. Release of Liability

I, the undersigned participant (or parent/guardian if the participant is under 18), acknowledge that participation in dance and fitness activities involves inherent risks, including but not limited to the risk of injury, illness, or accidents. I hereby release, discharge, and hold harmless Soul Studios, its owners, employees, instructors, and agents from any and all liability, claims, demands, or causes of action that may arise from or relate to my (or my child's) participation in any and all activities associated with Soul Studios. This release of liability applies to any and all damages or injuries, whether caused by negligence or otherwise.

I understand that it is my responsibility to consult with a physician prior to and regarding my (or my child's) participation in any dance or fitness program. I agree to assume full responsibility for any and all risks, injuries, or damages that may occur as a result of participation in the program.

2. 30-Day Notice of Cancellation

I understand and agree that if I (or my child) choose to terminate participation in the dance program, I am required to provide Soul Studios with a 30-day written notice of cancellation. If notice is not provided, I agree to pay the equivalent of one month’s tuition as a cancellation fee. This fee is non-negotiable and will be automatically charged to the payment method on file.

3. Monthly Tuition Payment

I understand that tuition for the dance program is due on the first of each month and is non-refundable. I agree to ensure timely payment of the monthly tuition to maintain my (or my child's) spot in the class. Failure to pay tuition on time may result in loss of class placement and the accrual of late fees. I acknowledge that tuition is not prorated for missed classes, holidays, or closures due to inclement weather or other unforeseen circumstances.

4. Photo and Media Release

I hereby grant Soul Studios the right to use photographs, video recordings, and other media taken during classes, performances, or other events for promotional purposes. This may include, but is not limited to, social media, website content, brochures, and other marketing materials. I understand that I (or my child) will not receive any compensation for the use of these images or media.

If I do not consent to this photo release, I agree to notify Soul Studios in writing before the first class.
