ABL FITNESS, INC. – ACCIDENT WAIVER & MEDIA RELEASE FORM

WAIVER & RELEASE OF LIABILITY

I, the undersigned, acknowledge and agree that participating in the ABL Fitness Powerlifting Invitational and any related fitness activities (including weightlifting, strength training, or aerobic activity) involves inherent risks. These may include, but are not limited to, personal injury, property damage, or death.

I certify that I am physically fit and capable of participating. I acknowledge that I have not been advised against participation by a qualified medical professional. I assume full responsibility for all risks associated with this event, including travel to/from the event, and release ABL FITNESS, INC., Karen Hurley, event sponsors, volunteers, staff, and the facility owners from any and all liability, claims, demands, or causes of action.

I further agree to indemnify and hold harmless the aforementioned parties from any and all claims resulting from participation, including those caused by negligence.
In the event of injury, I consent to receive medical treatment deemed advisable. This waiver is intended to be as broad and inclusive as permitted by Massachusetts law.

PHOTO & MEDIA RELEASE

I hereby grant permission to ABL FITNESS, INC. and its authorized representatives to capture, use, and publish photographs or videos taken of me or my child during this event. These may be used for promotional, educational, or marketing purposes in print or digital media (including social media and websites) without compensation or the right to inspect or approve the final product.

I release all claims related to the use of my image and affirm that ABL FITNESS, INC. retains all ownership rights to media created.

I certify that I am either 18 or older and signing for myself or I am the parent/guardian signing for a minor.
