Parent/Guardian & Adult Student Consent and Liability Waiver

By checking the box below and completing registration, I acknowledge and agree to the following:

I agree to pay all tuition and applicable fees according to MBT policies. I understand that tuition and registration fees for the Academic Year are non-refundable unless otherwise stated in writing by Metropolitan Ballet Theatre (“MBT”).
I agree to read, understand, and comply with all MBT policies, including the MBT Code of Conduct and studio procedures.
I understand that ballet and other forms of dance are physical activities involving inherent risks of injury. Participation in MBT classes, rehearsals, performances, camps, workshops, and related activities is voluntary, and I knowingly assume all risks associated with participation.
To the fullest extent permitted by law, I release and hold harmless Metropolitan Ballet Theatre, Inc., its directors, instructors, staff, volunteers, and affiliates from claims or liabilities arising from participation in MBT activities, except in cases of gross negligence or willful misconduct.
In the event of injury or illness, I authorize MBT staff to administer basic first aid and, if necessary, seek emergency medical care. I understand that every reasonable effort will be made to contact the parent/guardian or emergency contact promptly.
I acknowledge that I am financially responsible for any medical treatment provided.
I grant permission to Metropolitan Ballet Theatre, Inc. to photograph and/or record the student or adult participant and to use such images or recordings for educational, promotional, advertising, social media, website, press releases, and other lawful purposes related to MBT without compensation.
By checking this box, I acknowledge that I have read, understand, and voluntarily agree to the terms above. This electronic acknowledgment shall have the same force and effect as a handwritten signature.

September - June Classes
Registration for the full year classes is fully refundable less the initial monthly tuition deposit and registration fee until September 22th for year-long and Semester 1 classes, and February 10th for Semester 2 classes. The request to drop classes must be made in writing before the dates above. You are responsible for paying the full tuition amount after these drop dates whether or not your student attends the class. If you have an extenuating circumstance such as a serious illness or unexpected move, please email contact@mbtdance.org.

Summer Camps and Classes:
Camps and Classes are refundable until June 16th. Any summer camp or class dropped after June 16th will be refunded the tuition fee less a $100 administrative fee.

You are responsible for paying the full tuition, regardless of whether or not you attend classes.

Full Year Classes:
A 10% deposit and a once-per-Academic-Year registration fee of $50 are due at the time of registration. The balance will be divided into 7 monthly payments from September to March.

Please ensure a payment method is on file so we can set up payments.

MBT absorbs the processing fees if you pay by ACH (bank account). Credit/debit card payments will incur a 2.26% + transaction fee. You can add your bank account information to the portal to have the convenience of auto-pay without the extra costs.

If you wish to pay by cash or check, add the classes to your portal and then skip the payment part—email contact@mbtdance.org with the subject line PAYMENT ARRANGEMENT NEEDED.

If you want to pay in full by cash or check and receive a 2% discount, add the classes to your portal and then skip the payment part and email anneliese@mbtdance.org, with the subject line PAYMENT ARRANGEMENT NEEDED so we can assist you with registration.

Semester Classes:
A 10% deposit and a once-per-Academic-Year registration fee of $50 are due at the time of registration. The balance will be divided into 3 monthly payments from September- November.

Summer Classes and Camps:
Summer tuition must be paid in full at the time of registration, but is fully refundable through June 16th. email anneliese@mbtdance.org if you would like a more flexible option.
