Parent/Guardian and Adult Student Consent: I agree to pay the tuition, and to read and follow MBT Code of Conduct and liability policy. Ballet and other forms of dance are physical in nature, and participants and parents assume all risks and liabilities associated therewith. Parents/Guardians and Adult Students authorize MBT to administer first aid if necessary, and parents/guardians/emergency contacts will be informed as promptly as possible by MBT if injury or illness occurs. Parent/Guardians: I give permission for photos and/or video likenesses of my student to be used in publications, press releases, video productions, and website pages made by and for Metropolitan Ballet Theatre, Inc. Adult Students: I give permission for photos and/or video likenesses of my student to be used in publications, press releases, video productions, and website pages made by and for Metropolitan Ballet Theatre, Inc. I acknowledge that tuition during the Academic year is non-refundable.
Academic Year: Registration for the Academic Year is fully refundable less the initial monthly tuition deposit and registration fee until September 15th for year-long and semester 1 classes, and February 10th for Semester 2 classes. The request to drop classes must be made in writing before the dates above. You are responsible for paying the full tuition amount after these drop dates whether or not your student attends the class. If you have an extenuating circumstance such as a serious illness or unexpected move, please email contact@mbtdance.org. Summer Camps and Classes: Camps and Classes are refundable until June 16th. Any summer camp or class dropped after June 16th will be refunded the tuition fee less a $100 administrative fee.

Credit/debit card payments will incur a 2.26% + .30 transaction fee.
