I am the parent or legal guardian of the child(ren) listed above and give permission for my child(ren) to
participate in all activities offered by Upper Valley Cheer.
1. **Assumption of Risk:** I understand that cheerleading is a physical activity that includes tumbling, stunting, jumping, and lifting. These activities carry inherent risks, including falls, collisions, head or neck injury, sprains, broken bones, and in rare cases, serious injury or death. I voluntarily accept these risks on behalf of my child.
2. **Release of Liability:** On behalf of myself and my child(ren), I release and agree not to hold Upper Valley Cheer, its owners, coaches, staff, volunteers, or agents responsible for any injury, illness, property damage, or loss that
may occur as a result of participation, including those arising from the ordinary negligence of Upper Valley Cheer.
3. **Indemnification:** I agree to indemnify and hold Upper Valley Cheer harmless from any claims or legal actions brought on behalf of my child, except where prohibited by law.
4. **Emergency Medical Treatment:** In the event of an emergency, I authorize Upper Valley Cheer volunteer staff to obtain any necessary medical treatment for my children. I understand and accept responsibility for any incurred medical expenses.
5. **Photo and Video Release:** I give permission for Upper Valley Cheer to use photos or videos of my child taken during activities for promotional or educational purposes, including social media and marketing.
6. ** Medical Care & Health Responsibility:** I confirm that my child is physically able to participate in cheer activities and that I have disclosed any relevant medical conditions, allergies, or prior injuries. In the event of an emergency, I authorize Upper Valley Cheer to seek medical treatment for my child. I understand that I am responsible for all medical
expenses incurred.
7. ** Concussion & Safety Awareness:** I understand that cheerleading carries a risk of head injury or concussion. I agree to follow all safety guidelines, including medical clearance requirements, before my child returns to activity after an injury.
8. ** Coach Authority & Safety Rules:** I agree that all participants must follow gym rules and coach instructions at all times. Coaches have the final authority to determine skill readiness and may restrict participation or remove a child from an activity if safety rules are not followed.
9. ** Governing Law:** This agreement shall be governed by the laws of the state in which Upper Valley Cheer operates. If any part of this waiver is found to be unenforceable, the remaining sections shall remain in effect
