By signing below, you agree that you are aware that your child named above will be engaging in a physical activity offered by Triple Arts Studios. Classes offered are of a physical nature and include the risk of injury either from oneself or another participant or in the use of equipment and facilities, or of illness such as, but not limited to Influenza, MRSA, or COVID-19 that may lead to paralysis or death.

Triple Arts Studios has the right to combine or cancel classes if registration for the class is fewer than 3 students. If at any time it is required to cancel classes due to bad weather or instructor availability, families will be notified by email and/or text.
We will do our best to run classes except under dangerous driving conditions. Please use your own discretion regarding attendance if the driving is difficult in your area. If at any time it is required to cancel classes due to an emergency, an e-mail and/or text will be sent. If Triple Arts Studios cancels a class, a make-up class can be booked through the portal, a makeup date will be announced, or a credit will be applied for the missed class, at the Studio's discretion.

Karate: Students who are unable to attend a class can book their absence and makeup in the parent portal (please use the "drop in" option for makeups).
Karate students must commit to the full session. There will be no refunds for mid session cancellations.
Fitness and Theatre: Makeup classes are not available for the fitness and theatre programs.
Music: Makeup classes may be offered at the discretion of the instructor

PARENT RESPONSIBILITY - Teachers are only responsible for students during their class time. Parents are responsible for making sure their child arrives on time and are picked up on time. Please do not send students and/or siblings into the studio with a considerable amount of time unattended or if they do not have class. If a relative, friend, or neighbor is picking up your child, please inform staff ahead of time for safety reasons. Students under 12 years of age must have their parent or guardian present within the premises. Please try to keep missed classes to a minimum as students will be missing out on valuable training. No refunds will be given for missed classes.

STUDENT RESPONSIBILITY - Students are asked to be considerate and respectful to all teachers, parents and other students. Only water is allowed in the dojo/studio. Please no jewelry for karate students. Please be punctual. A ten-minute grace period will be accepted for latecomers. However, to avoid injuries (lack of warm-up) students in active programs that are later than 10 minutes will be asked to sit and watch the class. Students must wait inside the building for their ride to pick them up. Students must be in proper program attire for class and hair must be tied back securely. Cell phones: If a student decides to bring their cell phone into the dojo/studio, for security reasons, they will be required to place them in their bag. While classes are in progress, all cell phones must be off or on silent.

1. By registering for a Triple Arts Studios' class karate and theatre program(s), you are committing to the full season (September 1 to January 31, February 1 to June 30, July 1 to August 31). Music and fitness students enroll on a monthly commitment.
2. New students in the Karate program can enroll for a 4 week program, then transition for the balance of the session.
3. Fees are due upon registration in the form of an email transfer, cheque, cash or payment in the portal. Monthly payment plans are available upon request for session enrollments (Theatre and Karate).
4. If the full amount of the tuition is not paid within 30 days, 5% late fee is applied and the total session amount is due immediately.
5. Session fees are non-refundable. Memberships cannot be suspended.
6. A $29 fee will be charged for returned cheques.
7. Karate and Theatre Classes will not be pro-rated for absences, whether they are planned or unplanned. Pro-rated classes are available for new students only.
9. Music Lessons will only be credited if the front desk or administration is given 48 hours notification of absence. A makeup option may be offered at the instructor's discretion.

I agree that my or my child's picture or likeness can be represented and published in any Triple Arts Studios, Triple Arts Academy or Triple Arts Inc. publication or media, unless I have provided written instructions to the contrary.
