By registering for an account on the Oasis Dance Center parent portal, you agree to the following terms and conditions:

You must be at least 18 years old to register for an account on behalf of your child.
You are responsible for providing accurate and up-to-date information during the registration process.
You understand that the parent portal is intended for parental use to manage your child's dance activities, class schedules, and payments.
You agree to abide by the rules and policies set forth by Oasis Dance Center, including attendance, dress code, behavior, and payment guidelines.
You acknowledge that any misuse of the parent portal or violation of the terms may result in the suspension or termination of your account.

I understand and agree that participating in any dance class, or workshop, is a possibility of physical injury. I agree to release and hold harmless The Oasis Dance Center including its teachers, dancers, and staff members, from any cause of action, claims, or demands now and in the future. I will not hold The Oasis Dance Center liable for any personal injury or any personal property damage, which may occur on the premises before, during, or after classes. Furthermore, I understand that I should be aware of my and/or my child’s physical limitations and agree not to exceed them. I am signing this waiver for myself and/or my child, I certify that I am over 18 years of age or the parent or legal guardian and have the right to waive these rights.

By signing the waiver you acknowledge that you have understood and agreed to the terms of the waiver.
I CERTIFY THAT I HAVE READ THIS DOCUMENT, AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT OF MY OWN FREE WILL.
PARENT / GUARDIAN WAIVER FOR MINORS (Only if student is under 18 years old) The undersigned parent and natural guardian does hereby represent that he/she is acting in such capacity, has consented to his/her child or ward's participation in the activity or event, and has agreed individually and on behalf of the child or ward, to the terms of the accident waiver and release of liability set forth above. The undersigned parent or guardian further agrees to save and hold harmless and indemnify each and all of the parties referred to above from all liability, loss, cost, claim, or damage whatsoever which may be imposed upon said parties because of any defect in or lack of such capacity to so act and release said parties on behalf of the minor, the parents, of the legal guardian.
