NuEvolution Dance Studio regularly photographs and records dancers during classes, rehearsals, performances, competitions, conventions, studio events, and other dance-related activities.

These photos and videos may be used to celebrate dancer achievements, promote studio programs, share studio activities, and showcase the NuEvolution dance community.

1. Photography & Video Authorization
By granting permission below, I authorize NuEvolution Dance Studio, its employees, instructors, and authorized representatives to photograph, video record, and otherwise capture my dancer's image, likeness, and performance during studio-related activities.

2. Use of Photos & Videos
With my consent, NuEvolution Dance Studio may use these images and recordings for the following purposes:
- Official studio social media accounts, including Facebook, Instagram, TikTok, and other platforms.
- NuEvolution Dance Studio's website and online promotional materials.
- Studio advertisements, flyers, brochures, and marketing campaigns.
- Recognition of dancer achievements, competitions, performances, and special events.
- Studio newsletters, emails, and parent communications.
- Promotional videos, recital highlights, and studio presentations.
I understand that approved photos and videos may be viewed, shared, or reposted by others once published online.

3. Privacy & Protection of Dancers
NuEvolution Dance Studio will make reasonable efforts to protect dancers' privacy and use images and recordings appropriately.
The studio will not intentionally publish a dancer's personal contact information, home address, or other sensitive identifying information.
The studio may identify dancers by first name when celebrating achievements, unless a parent requests otherwise.

4. Consent & Withdrawal
I understand that:
- Granting promotional photo and video permission is voluntary and is not required for enrollment.
- I may opt out or withdraw my permission for future promotional use by going to setting in my parent portal.
- Withdrawal does not require the studio to retrieve printed materials already distributed or remove content shared or reposted by third parties.
- The studio will make reasonable efforts to remove previously published content from studio-controlled platforms upon written request.

I acknowledge that I have read and understand this Photo, Video & Social Media Consent Agreement and have indicated my permission preference above.

Studio Fees are for the season beginning July/August - May. *Fees do not apply to Summer.
• A non-refundable registration fee of $40 for the first dancer and $10 each additional dancer up to a max of $70.
• A non-refundable costume deposit of $75 is due by October 1st. Costume balance will be due when the costume arrives at the studio. All costume balances need to be paid in full to
pick up your dancer's costume. Please notify the teacher or front desk if your child is NOT participating in the May Recital. Once costumes are ordered you will be responsible for the
full costume amount.
• There is a $60 recital fee, due May 1st to participate in the recital.
*There is a $25 fee for returned checks. This is fee is applied year round.

• I understand that tuition is due in full by the 1st of every month.
• NuEvolution will prorate tuition if your dancer joins after the first week of the month.
• Account balances will be subject to a $15 late fee if paid after the 10th of each month.
• Tuition is a yearly rate, broken down into a monthly fee. Tuition is the same each month regardless of the number of weeks and holiday closures.
• You are responsible for tuition up until the date you notify us to unenroll your dancer. Please notify us if your dancer will no longer be attending class.
• Payments are applied to the oldest balance first.
• If your account is more than 30 days past due, your child will be dropped from class(es) and not be allowed to participate until the balance is paid in full.
• Account reaching 90 days past due will be sent to collections.
• All accounts must be paid in full to participate in recital.
• I understand that NuEvolution Dance Studio does not give credits and or refunds for missed class(es) due to vacation, illness, weather, ect. Make up class(es) may be taken in any age appropriate class within 30 days of the missed class. Dancers may take the same class or any style offered for their age.
