I voluntarily agree to release, waive, discharge, hold harmless, defend and indemnify FCDAC and its directors, officers, employees, agents and volunteers from any and all claims, actions or losses for bodily injury, property damage, wrongful death, loss of services or otherwise which may arise directly or indirectly as a consequence of participating in FCDAC activities, which include but are not limited to: outdoor performances, indoor performances, in and out of city competitions, and practices, whether arising from FCDAC’s negligence or otherwise. I specifically understand that neither I, nor my child, will have any rights against FCDAC arising out of participation in their activities. I authorize FCDAC to use photographs or videos of me or my child for promotional or educational purposes in any type of media, including their websites. I understand that I will not be paid or rewarded for providing this authorization.

I HAVE RAED AND UNDERSTOOD THE ABOVE WAIVER AND RELEASE AND BY SIGNING IT AGREE AND ACKNOWLEDGE THAT IT IS MY INTENTION TO EXEMPT AND RELIEVE FOLK CREW DANCE ACADEMY CALGARY FROMLIABILITY FORPERSONAL INJURY, PROPERTY DAMAGE OR WRONGFUL DEATH CAUSED BY NEGLIGENCE OR ANY OTHER CAUSE IN THE PARTICIPATION OF THEIR ACTIVITES. I FURTHER ACKNOWLEDGE THAT FOLK CREW DANCE ACADEMY CALGARY IS RELYING UPON THIS WAIVER AND RELEASE IN ITS PROVISION OF ACTIVITES FOR MY OR MY CHILD’S USE.
