I/we, realize that participation in dance classes and activities could involve some possible personal injury. By signing this release form, I/we assume all risks related to the use of any and all spaces used by Nantucket Dance Theater Inc. I further state that I have consulted with a physician regarding my child’s physical health, my child is physically fit, and I assume responsibility for the risk of injury to my child.

I/we agree to release and hold harmless Nantucket Dance Theater Ince including its teachers, dancers, staff members, and facilities used by both entities from any cause of action, claims, or demands now and in the future. I/we will not hold Nantucket Dance Theater Inc, liable for any personal injury or any personal property damage or loss, which may occur on the premises before, during, or after classes.

We would like your permission to use photographs taken during classes, or performances to post for publicity purposes. We promote Natucket Dance Theater and Nantucket Dance Festival using color brochures, on our website, social media, and other types of promotional materials. When photographs are used for publicity purposes, children are never identified by name. All photos used for publicity will be available for the child’s parents to review upon request. In addition, parents reserve the right to request that any photograph not be used for publicity
