You must be over 18 to register for an account with on Pointe Dance Centre, or have a parent or guardian present while the registration form is being filled out.

I understand that in some cases, or with certain health conditions, physical movement can lead to injury or other serious medical conditions. I recognize that participation in classes conducted at On Pointe Dance Centre is done at my own risk and the risk of my child. I therefore waive and release, discharge and hold harmless On Pointe Dance Centre, and any or all its representatives from any liabilities that may arise from or be related to the classes at On Pointe Dance Centre. I further agree to hold harmless On Pointe Dance Centre and any or all its representatives from any liabilities by my negligence or willful misconduct, or that of my child.

*I give On Pointe Dance Centre permission to use pictures/videos of my children for their website, Facebook and other advertising mediums only for the use of promotion for On Pointe Dance Centre.

*I understand and acknowledge the Picture/Video policy in order to protect and respect the privacy and property of On Pointe Dance Centre’s students, facility, and staff that I, myself, my spouse, or my child, may not publish any pictures or videos of On Pointe Dance Centre’s students, staff, teachers and choreography on the internet. This includes but not limited to websites such as Facebook, YouTube, Instagram, and Twitter. I do understand that I may publish pictures of my own children on my personal websites as I wish.

By enrolling your dancer at On Pointe Dance Centre, you acknowledge and agree to the following terms regarding tuition and fees:

1. Understanding of Fees:
The parent or legal guardian of the enrolled dancer confirms they have reviewed and understand the tuition structure and associated fees for dance classes as outlined by On Pointe Dance Centre. This includes, but is not limited to, monthly tuition, registration fees, costume fees, recital fees, and any additional charges applicable to their selected classes or programs.

2. Payment Responsibility:
The parent or guardian agrees to be responsible for timely payment of all tuition and fees. Payments are due in accordance with the schedule provided during registration and must be kept current to ensure the dancer’s continued participation.

3. Non-Refundable Fees:
All registration and costume fees are non-refundable. Tuition is also non-refundable, except in cases where the studio cancels a class and no alternative option is available.

4. Late Payments:
Late payments may incur additional fees as detailed in the payment agreement provided during registration. The studio reserves the right to suspend the dancer’s participation if accounts are not kept up-to-date.

5. Agreement to Terms:
By enrolling their dancer, the parent or guardian acknowledges their understanding of this policy and agrees to adhere to its terms.
