I hereby give permission for my child (the Performer listed in this registration) to participate in the Stage Right Performing Arts, LLC (SRPA) Show Choir Program. I hereby release SRPA (its contractors, employees, and volunteers) as well as any venue in which SRPA holds rehearsals or performances, and all other participants from any and all liability for any injuries and/or damages that may be sustained by my child as a result of my child’s participation in the SRPA Show Choir Program. I further give my permission for the use of my child’s likeness by SRPA in any and all formats, including photos, videos, and/or sound recordings, for use in SRPA’s printed materials, recordings and/or website. I understand that the use of such likeness shall not be compensated and that my child’s appearance in any such materials and/or website does not confer any rights of ownership. If my child is selected to participate in the SRPA Show Choir Program and he/she desires to join, I understand an annual registration fee (fees based on choir) is due with this completed registration form and a choir tuition is due in 5 payments, as described in the most current SRPA Show Choir Handbook, which governs participation in the SRPA Show Choir Program. I understand that any alternate financial arrangements must be made in advance, in writing with the Executive Director of SRPA.

I hereby release SRPA (its contractors, employees, and volunteers) as well as any venue in which SRPA holds rehearsals or performances, and all other participants from any and all liability for any injuries and/or damages that may be sustained by my child as a result of my child’s participation in the SRPA Young Professionals Program. I further give my permission for the use of my child’s likeness by SRPA in any and all formats, including photos, videos, and/or sound recordings, for use in SRPA’s printed materials, recordings and/or website. I understand that the use of such likeness shall not be compensated and that my child’s appearance in any such materials and/or website does not confer any rights of ownership. If my child is selected to participate in the SRPA Young Professionals Program and he/she desires to join, I understand an annual registration fee (as noted above) is due with this completed registration form and a tuition is due either as an annual payment or in a payment plan, as described in the registration information above. I understand that any alternate financial arrangements must be made in advance, in writing with the Executive Director of SRPA.
