LDA Liability-Release

* I understand LDA is not responsible for the above student pertaining to any accident, injury, or theft at the studio. Students are only supervised in the dance studio during class time. The LDA is not responsible for students in the lobby or outside the building.

* I agree TO ASSUME AND ACCEPT ALL RISKS associated and related to my/minor’s
participation in the program, even though such risks may lead to injury. This
assumption and acceptance of risks includes any medical expenses that I/minor
may incur as a result of personal injury or illness.

*I understand and acknowledge that Ludovici Dance Academy Directors, officers,
employees, teachers, staff members, choreographers, assistants, helpers, and
volunteers including guest faculty & Independent Teachers, are not responsible for
any injury, death, loss, theft, or damage sustained by me/minor while participating
in the program however caused on the property of the Ludovici Dance Academy.

*I agree to pay the non-refundable registration fee and that skipping a month of class,
results in extra re-entry fees. I understand the fees associated with tuition payments and agree to pay in full for all classes registered for. Payment discounts on 'month ahead' rates are non-refundable and credit only.

*I understand rates are based on multiple attendance/class discounts and that failure to fully attend all classes will not result in reduced tuition nor result in payment refunds. I understand that most (but not all) classes are provided with make up options for any and all missed classes due to injury, illness, vacation, or studio Holiday breaks.

*I understand that should I/minor or my parent/guardians choose to quit at any time or have action that results in removal of me/minor from the program associated with any of the following: illegal behavior or any obscene behavior including disrupting behavior and/or resulting in damage of any kind to another and/or severe disrespect to a faculty member or other student, I cannot get fees back for any classes, costumes (weather worn or not), or other fees paid in advance for an activity I am no longer to participate in.


*I Understand the costume for the end of year show is ordered in the fall. Should I withdraw from class after Thanksgiving, I will not receive a reimbursement for the costume that is ordered for the show.

*I understand it is required to leave a credit card on file and anytime during the duration of the dance year, should my monthly tuition be unpaid, my card may be charged for the full amount of tuition + any added late fees.
Should my child at any time not return to dance, I understand there is an official “class withdraw” paper I need to fill out from the office. Failure to officially withdraw from any class or classes will result in continual monthly tuition until the class or classes have officially been withdrawn from and turned into front desk.

Note: Automatic credit card withdraws for unpaid tuition will not be made until the LAST FRIDAY of each month. Later fees are applied to payments after the 15th of the month. Early bird discount rates are for payments made the month AHEAD.

*I understand that should we commit to the closing "Gala" show, the student is expected to attend all
designated Staging & show rehearsals. THESE DO INCLUDE SATURDAY, AND SUNDAY prior or "Memorial weekend'.
SHOW DAY: Sunday June 7th
Dress + Tech rehearsal Saturday June 6th
Dress Rehearsals: Friday, June 5th

Staging rehearsals:
BEGIN: FRIDAY MAY 29TH, SAT. MAY 30, SUN MAY 31
MON JUNE 1, TUES JUNE 2, THURS. JUNE 4

- NO REH Wed. June 3rd + NO REH MEMORIAL WEEKEND!!!!


*I understand that any expenses associated with the Dance Show: dance including lessons, costumes, Staging/rehearsal fees, program fees, tickets are NON-REFUNDABLE for any reason, this includes deposits which occur throughout the year.
