I am aware that participation in Élevé Studio of Dance classes, rehearsals, community events, and recital events have some inherent risks and injury can occur. On rare occasions these injuries can be serious. In consideration of my child being allowed to participate in the aforementioned activities, I, the parent/guardian, by signing below, assume the risk of all injury, agree not to sue, and to hold harmless Élevé Studio of Dance, the event holders, directors, instructors, employees, and volunteers for any and all injuries caused by or resulting from participating in Élevé Studio of Dance classes, rehearsals, community events, and recital events.
Caregivers, legal guardians of minor students, and adult students waive the right to any legal action for any injury sustained on studio property resulting from normal dance activity or any other activity conducted by the students before, during, or after class time. I authorize ESD staff to seek emergency medical help if this becomes necessary. I realize that every effort will be made by ESD staff to contact me in the event of a medical emergency involving my child, and I agree to indemnify and hold harmless ESD personnel in seeking medical care for my child.
