By participating in dance classes, rehearsals, performances, and any other activities associated with Project 3 Dance Company, dancers and their parents/guardians acknowledge and accept the inherent risks involved. These activities may include, but are not limited to, physical exertion, stretching, jumping, and other strenuous movements that can potentially lead to injury. Project 3 Dance Company, its instructors, staff, and affiliates are not liable for any injuries sustained while participating in studio activities. Participants and their guardians agree to assume full responsibility for any and all risks, known and unknown, associated with these activities, and release Project 3 Dance Company instructors, staff, and affiliates from any claims or liabilities arising from participation. It is strongly recommended that participants have appropriate medical insurance coverage.

By enrolling in classes at Project 3 Dance Company, I understand and agree to the following financial responsibilities:

- Tuition and Fees: Tuition payments are due on the first day of each month via CASH OR ZELLE ONLY, with a 5-day grace period. Payments made after the 5th of each month (11:59 PM) will incur a $35 late fee.
- Refund Policy: Refunds will not be offered once fees have been paid, even if a dancer chooses to withdraw from the program.
- Costumes and Additional Fees: I am responsible for all additional costs, including but not limited to costumes, competition fees, and special events.
- Fundraising: Participation in fundraising activities is optional and does not alter my obligation to pay tuition and other fees on time.
- Payment Methods: Accepted payment methods include cash and Zelle only.
By signing below, I acknowledge and agree to these terms and accept full financial responsibility for all associated costs of participating in Project 3 Dance Company programs.

I hereby grant Project 3 Dance Company, its representatives, and employees the right to take photographs and video recordings of my child in connection with dance classes, performances, and other related activities. I authorize Project 3 Dance Company to use and publish these images and videos in print and/or electronically.

I agree that Project 3 Dance Company may use such photographs and video recordings with or without my child's name and for any lawful purpose, including but not limited to publicity, promotional materials, social media, advertising, and web content.

I have read and understand the above photo and video release. I agree to allow my child’s photos and videos to be used as described.

I, the undersigned dancer (or parent/guardian if dancer is under 18 years old), acknowledge and fully understand that participation in dance classes, rehearsals, and performances at Project 3 Dance Company involves physical activity that carries the risk of injury. I hereby release, discharge, and hold harmless Project 3 Dance Company, its directors, instructors, employees, and agents from any and all claims, demands, or causes of action that may arise from or relate to any injury, damage, or loss incurred during participation in any dance activities, whether caused by the negligence of Project 3 Dance Company or otherwise.

I understand and agree that it is my responsibility to consult with a physician prior to and regarding my (or my child’s) participation in dance activities. I certify that I (or my child) am physically fit and have no medical conditions that would prevent full participation in the dance program.

By signing this release, I acknowledge that I have read, understood, and agree to the above terms and conditions.
