PAYMENT POLICIES & PROCEDURES I, the undersigned parent or legal guardian of the above stated student, have read and fully understand the following policies and procedures of Artistry Dance Lab, LLC: 1. Participation for the dance year at Artistry Dance Lab, LLC is predicated on payment of tuition which is billed on a monthly basis. 2. Tuition is based on a 9-month dance year from September 8, 2025, to May 17, 2026, for recital and technique classes; 3. Monthly bills are due and payable on or before the first day of each month through our auto-pay system. Any and all late payments are subject to a $25 late fee that will be applied to the following months billing. 4. There is a $35 fee for all checks that are returned due to insufficient funds. 5. In addition to Artistry Dance Lab, LLC's dance year tuition there will be separate fees for recital costumes that are to be paid by the student. All recital costume fees must be paid in full by December 16th, 2025. (2 payments of $40 due on 10/19/25 and 12/16/25 for each recital class.  Competition costumes will be billed separately.)  Any and all late payments of recital costume fees are subject to a $10 late fee. 6. Students who drop class after 10/19/25 will be assessed a $75 charge per class dropped. 7. All students, parents, guardians and guests agree to abide by all rules while at Artistry Dance Lab. I understand that by signing below I, the parent/guardian of the above-named student, am agreeing to be financially responsible for the costs associated with the student's participation, as outlined above. I understand that all payments made for tuition, costumes, and competition fees are non-refundable. I further understand that these policies may be supplemented over the course of the dance year and that I will be notified of any such supplements in writing.

I the parent/legal guardian of the above named student desire to release certain parties from any damages that may arise, directly and indirectly, as the result of the student's participation in the activities offered by ARTISTRY DANCE LAB, LLC, fully realizing the inherent danger in any type of activity of this nature and that those dangers include the RISK OF INJURY and even DEATH. Furthermore, In the event of injury to my child, if or when my spouse or I cannot be contacted I give permission to the attending physician to render treatment as would be normal, and I agree to pay the usual charge for such treatment. In consideration of the student being permitted by ARTISTRY DANCE LAB, LLC to participate in the activities connected to dance training and performance. I for the student and the student's parents, heirs, assigns and personal representatives hereby RELEASE AND DISCHARGE ARTISTRY DANCE LAB, LLC, its owners, officers, directors, contractors, agents, employees and volunteers from any and all claims, actions, suits, proceedings, costs, expenses, damages and limitations, resulting from any injuries or damages to the student. I also agree to RELEASE AND DISCHARGE ARTISTRY DANCE LAB, LLC its owners, officers, directors, contractors, agents, employees and volunteers from any and all claims, actions, suits, proceedings, costs, expenses, damages and limitations incurred by any third party, due to the student's actions either negligent or otherwise. I the parent/legal guardian of the student agree for myself and my heirs, distributees, guardians, legal representatives and assigns to indemnify ARTISTRY DANCE LAB, LLC, its owners, officers, directors, contractors, agents, employees and volunteers, in the event of any and all claims for personal injury, death or property damage, including any reasonable attorney fees incurred by ARTISTRY DANCE LAB, LLC its owners, officers, directors, contractors, agents, employees and volunteers, in defending said claims. I have carefully read this Contract for Release of Liability. I fully understand its contents. I am aware that this is a RELEASE OF LIABILITY. I sign it of my own free will.
