I understand and agree there are risks of physical injury associated with, arising out of and inherent to the activity of dance class, workshop, rehearsal, performance and the like. In recognition of this acknowledged risk of injury, I knowingly and voluntarily indemnify and waive all right and/or caused of action of any kind whatsoever including any and all claims of negligence arising as a result of such activity from which liability could accrue instructors, subsidiaries, parent corporations and all affiliated entities (hereinafter referred to as “Serykh Conservatory of Dance LLC”).

I hereby agree to release Serykh Conservatory of Dance LLC and hold the same harmless of all liability, claims demands, or causes of action whatsoever from any damage, loss, injury, or death to me, my child, or property which may arise out of or in connection to participation in any classes or activities conducted by Serykh Conservatory of Dance LLC, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury arising out of participation in classes, workshops, rehearsal, performance or any other activity on behalf of the participant.I hereby voluntarily agree to waive my rights and that of my heirs and assigns to hold the Serykh Conservatory of Dance LLC its owners, agents, volunteers, assistants, employees, guest artists, instructors, and/or students liable for such damage, loss, injury or death. I understand that I should be aware of my physical limitations and agree not exceed them.

I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing assumption of risk of injury. I have signed this document voluntarily and of my own free act and deed in exchange for participation in activities at Serykh Conservatory of Dance LLC.

If I am a minor, my parent and/or legal guardian has also signed this document releasing Serykh Conservatory of Dance LLC from any and all such liability described above and has acknowledged that I am knowingly and voluntarily assuming all risks of injury inherent to this activity.

The participant has parental permission to participate in activities at Serykh Conservatory of Dance LLC. I warrant the below information is complete and correct. I further release Serykh Conservatory of Dance LLC of all liabilities associated with my child’s attendance at Serykh Conservatory of Dance LLC.

Permission is granted to Serykh Conservatory of Dance LLC to use photographs of participants for publicity purposes.

I acknowledge that I have read the above Acknowledgement of Risk and Release of Liability and agree to be bound by the terms stated herein.

Current tuition rates are posted at the studio and on our website. Tuition is payable in advance and is due on the 1st of each month.

For your convenience, families may enroll in automatic payments (Auto-Pay). Auto-Pay payments will be automatically charged to the payment method on file on the 5th of each month.

Make-up classes are encouraged and may be taken at any time during the current semester in a class of the same level or, when necessary, a lower level. No credits or refunds will be issued for missed classes.

We accept cash, checks and credit/debit cards. A 3% processing fee will be added to all credit/debit card payments.

Payments received after the 15th of the month will be assessed a $15 late fee. Returned checks will be subject to a $25 returned check fee.

By signing below, I acknowledge that I have been informed of, understand and agree to the tuition and billing policies of Serykh Conservatory of Dance.
